a) WAR GASES
Definition and Terminology
The term "war gases" is technically a misnomer - not all substances used are true gases, nor are they used exclusively in warfare. The term broadly refers to any chemical agent deployed to incapacitate, injure, or kill adversaries. These substances may be true gases, smokes, vaporized liquids, or fine powders, used both in warfare and for dispersing unruly mobs.
Qualities of an Ideal War Gas
- Capable of cheap large-scale manufacture, preferably as an industrial by-product
- Definitely toxic even at low concentrations, effective over a wide area
- Heavier than air so it is not easily dispersed
- Sufficiently volatile to pervade the terrain under attack
- Chemically stable and non-corrosive to storage containers

Lacrimators: Fired in artillery shells; cause intense eye irritation, severe lacrimation, eyelid spasm, and temporary blindness. Treatment - irrigate eyes with normal saline; apply sodium bicarbonate to nasal/facial irritation.

Lung irritants: Chlorine and phosgene are gases (liquids under pressure) released from tanks or shells. Cause dyspnoea, chest tightness, coughing, cyanosis, and death within 1-2 days from acute pulmonary oedema or bronchopneumonia. Gas mask is protective. Treatment - fresh air, oxygen, cardiac stimulants, codeine for cough, antibiotics. Autopsy shows markedly oedematous/cyanotic lungs, foam in airways, pleural effusion.

Vesicants: Mustard gas smells of garlic; lewisite smells of geraniums. Mustard gas penetrates clothing, causes intense itching blisters and ulcers in moist skin areas, and damages eyes/respiratory tract. Lewisite rapidly blisters skin, inflames mucous membranes; after absorption causes haemolysis and arsenic-like toxicity. Gas mask protects from inhalation but not skin burns. Treatment - remove contaminated clothes, wash with soap and water. BAL is the antidote for lewisite.

Nerve agents: Block acetylcholinesterase, preventing transmission of nerve impulses, leading to paralysis and death.

Historical use: First large-scale deployment was chlorine gas by Germany in 1915 (WWI). Mustard gas, first used in 1917, was the most infamous WWI agent - its delayed action (symptoms appearing hours after exposure) made it particularly dangerous.
b) HYDRARGYRISM (Chronic Mercury Poisoning)
Definition
Hydrargyrism is chronic poisoning by mercury. It may result from:
- Continuous occupational absorption by workers
- Accidental exposure
- Excessive or prolonged therapeutic use
- Recovery from a large acute dose
- Prolonged use of mercurial ointment as an external application
Clinical Features
Classic Triad: Gingivitis + Stomatitis + Salivation, Tremors, and Neuropsychiatric changes.
Detailed features include:
- Salivation, inflammation of gums, occasional blue line at the gum-tooth junction
- Sore mouth and throat, loosening of teeth
- Gastrointestinal disturbances, anorexia, weight loss
- Anemia
- Chronic nephritis with progressive uraemia
Tremors:
- Begin in the hands, then progress to lips, tongue, arms, and legs
- Moderately coarse, interspersed with jerky movements
- Called "Danbury tremors"
- Advanced stage known as "Hatter's shakes" or "Glass-blower's shakes" (common in hat-making and glass-blowing industries)
- Most severe form: Concussio mercurialis - no voluntary activity is possible
Mercurial Erethism (seen in mirror-manufacturing workers):
Psychological effects including anxiety, depression, shyness, timidity, irritability, loss of confidence, mental depression, delusions, hallucinations, suicidal melancholia, manic-depressive psychosis ("Mad Hatter syndrome"), emotional instability, memory loss, and insomnia.
Ocular Changes:
Brownish deposit of mercury on the anterior lens capsule visible on slit-lamp examination (malt-brown reflex). Bilateral but does not affect visual acuity.
Renal Changes: Membranous glomerulonephritis with hyaline and fatty casts in urine. The kidney is the primary target organ.
Acrodynia (Pink Disease):
An idiosyncratic hypersensitivity reaction (especially in children) to chronic mercury exposure. Features: anorexia, insomnia, sweating, generalized skin rash, photophobia. Hands and feet become puffy, pinkish, painful, and paraesthetic with peeling skin. Teeth may be shed.
Treatment of Hydrargyrism:
- Remove patient from further exposure
- Demulcents, saline purgatives, oral hygiene
- Chelation therapy (drug of choice: D-penicillamine 25-40 mg/kg/day in children; 250 mg 4 times/day in adults - until urine mercury < 50 μg/L)
- Alternatively: BAL 100 mg IM 4th hourly for 2 days, then 100 mg every 8 hours for 8-10 days
- DMPS or DMSA may also be used
- Note: Ca-EDTA should NOT be used (nephrotoxic with mercury); BAL + penicillamine combined should NOT be used (may form toxic compound)
Minamata Disease: A special form of organic mercury poisoning from consuming mercury-contaminated fish, which occurred as a disaster in Japan in 1956 (Minamata Bay).
c) RECTIFIED SPIRIT
Definition
Rectified spirit (also called neutral spirit) is 90-95% ethyl alcohol. It is a purified form of ethyl alcohol obtained by repeated distillation ("rectification") of fermented liquor to remove impurities and concentrate the alcohol.

- Used commercially and medically as a solvent, antiseptic, and preservative
Medico-legal Uses of Rectified Spirit:
- Preservation of viscera for chemical analysis in cases of suspected poisoning - viscera are routinely preserved in rectified spirit (or saturated saline solution)
- Exceptions: When poisoning by carbolic acid, alcohol, phosphorus, paraldehyde, or acetic acid is suspected - saturated common salt solution is used instead (to avoid destroying the poison)
- Urine may also be preserved with an equal quantity of rectified spirit
- Blood and urine are preferably preserved in sodium fluoride
Distinction from Related Substances:
- Methylated spirit / Denatured spirit: Alcohol denatured with 9.5% methyl alcohol + 0.5% crude pyridine - rendered unfit for drinking; used industrially

d) LOVE PHILTER
Definition
A love philter (also spelled "love filter" or "love potion") is a drug or substance that is supposed to increase the feeling of love between the giver and the recipient. In a medico-legal context, love philters are important because they represent a recognized cause of accidental poisoning - people unknowingly administer or consume toxic substances believing them to be aphrodisiacs.
Medico-legal Significance:
- Love philters fall under accidental poisoning in Indian forensic medicine
- They are often given surreptitiously by one person to another to induce affection
- Many plants and drugs used as love philters are genuinely toxic at the doses administered
- Quack remedies containing poisonous drugs are frequently involved
