Most likely diagnosis: Central serous chorioretinopathy (CSCR)

This presentation is most consistent with right-eye central serous chorioretinopathy, also called central serous retinopathy, provided that a dilated retinal examination and OCT demonstrate subretinal fluid at the macula.
CSCR is a leakage disorder involving the retinal pigment epithelium and choroid. Fluid collects beneath the central retina (macula), creating a shallow serous elevation that causes persistent central blur and sometimes distortion.

Typical associated symptoms to ask about are distorted straight lines (metamorphopsia), a central gray/dark spot, reduced contrast or color saturation, and objects appearing smaller (micropsia). Kanski describes CSCR as unilateral blurring with metamorphopsia, micropsia, and mild color disturbance, caused by localized serous detachment at the macula. Kanski's Clinical Ophthalmology, 10th ed., p. 616-617.

Important limitation

A diagnosis cannot be confirmed from visual acuity and anterior eye findings alone. A clear cornea only tells us the cause is unlikely to be corneal. The decisive information is at the back of the eye.
The patient needs:
  1. Dilated fundus examination
  2. Macular OCT, which should show a shallow collection of fluid below the neurosensory retina, sometimes with pigment epithelial detachment
  3. Amsler grid, for central scotoma/distortion
  4. Fluorescein angiography, if the OCT/fundus appearance is atypical or to exclude mimics. It may show classic focal leakage with an “ink-blot” or “smoke-stack” pattern. The Wills Eye Manual, p. 827-830.
The AAO description of CSCR also notes that OCT and, where necessary, fluorescein angiography help identify fluid and rule out similar retinal disorders.

Differential diagnosis

1. Central serous chorioretinopathy - most likely

Expected retinal findings: a round or oval shallow macular serous detachment, usually without subretinal hemorrhage or lipid exudates. The Wills Eye Manual, p. 827.

2. Uncorrected refractive error or accommodative problem

This is common and must be checked with refraction and pinhole testing. However, a new unilateral central blur falling to 20/100 over 2 months deserves retinal imaging even if refraction improves some acuity. CSCR itself can induce a temporary hyperopic shift because elevated retina changes the optical focus.

3. Macular edema

This includes hypertensive retinal vascular disease, retinal vein occlusion, inflammatory edema, or medication-associated edema. Hypertension makes vascular disease relevant. Fundus examination and OCT would show retinal thickening/cysts rather than the typical isolated subretinal fluid pattern of CSCR.
A retinal vein occlusion usually has retinal hemorrhages, venous dilation/tortuosity, and cotton-wool spots. The absence of a visible scleral hemorrhage does not assess this. The retina must be dilated and examined.

4. Macular hole or vitreomacular traction

Can cause unilateral central blur, distortion, and reduced acuity. OCT distinguishes it clearly by showing a foveal defect or tractional change rather than subretinal fluid.

5. Choroidal neovascularization or early macular degeneration

Less likely at age 42 and in the absence of drusen/hemorrhage, but it must be excluded, especially if there is subretinal blood, lipid, pigment changes, or persistent/recurrent fluid. CSCR can sometimes resemble neovascular macular disease. The Wills Eye Manual, p. 828-829.

6. Hypertensive retinopathy or hypertensive choroidopathy

Controlled hypertension is less suggestive, but blood pressure should be measured now. Acute severe hypertension may cause retinal hemorrhages, cotton-wool spots, optic disc edema, or serous retinal detachments. This would be more concerning if BP is markedly elevated or there are headache, neurologic symptoms, chest pain, or kidney dysfunction.

7. Optic neuritis or other optic neuropathy

Less likely because optic neuritis commonly causes pain on eye movement, reduced color vision, and often a relative afferent pupillary defect. It can produce central visual loss, but the history here lacks its typical painful inflammatory pattern. Optic nerve assessment, color testing, and pupillary examination remain necessary.

8. Retinal detachment, optic-disc pit maculopathy, choroidal mass, or inflammatory choroidopathy

These are less common but important retinal causes of subretinal fluid. An optic-disc pit, retinal break/detachment, choroidal hemangioma/melanoma, posterior scleritis, and inflammatory conditions can mimic CSCR. The Wills Eye Manual, p. 828-829.

Targeted history that could support CSCR

Ask specifically about:
  • Any corticosteroid exposure, including oral tablets, injections, inhalers, nasal sprays, skin creams, or eye drops
  • Major psychological/physical stress and poor sleep
  • Obstructive sleep apnea symptoms
  • PDE-5 inhibitors, such as sildenafil/tadalafil
  • Cushingoid features or endocrine symptoms
  • Pregnancy, if applicable
  • Previous episodes in either eye
Steroid exposure should not be stopped abruptly without the prescribing clinician, but it should be reviewed urgently because corticosteroids can precipitate or worsen CSCR. CSCR has recognized associations with steroids, systemic hypertension, stress, and sleep apnea. Kanski's Clinical Ophthalmology, 10th ed., p. 616.

Clinical conclusion

Working diagnosis: right-eye central serous chorioretinopathy, pending macular OCT and dilated retinal examination.
At two months with vision of 20/100, this should be assessed promptly by an ophthalmologist, preferably a retina specialist, rather than simply observed without imaging. Many acute cases improve spontaneously within 3 to 6 months, but persistent subretinal fluid can damage photoreceptors and the retinal pigment epithelium. Kanski's Clinical Ophthalmology, 10th ed., p. 617.
Seek same-day emergency ophthalmic assessment if there is sudden worsening, flashes/floaters, a curtain-like field defect, new severe headache with very high BP, new neurologic deficit, or marked loss of color vision.
