What This Diagram Illustrates - Biological Process

Establishment of Left-Right Asymmetry (Lateralization)
During gastrulation, the primitive node contains monocilia that rotate in a clockwise direction when viewed from above, generating a leftward fluid flow called "nodal flow." This sweeps morphogens (particularly FGF8 and Nodal) to the left side of the embryo, activating the signaling cascade above.
Outcome:
  • Left side: Heart, stomach, and spleen develop on the left; the heart loops to the left
  • Right side: Liver and gallbladder develop on the right
  • PITX2 is the final downstream effector that drives these asymmetric organogenesis events
Clinical relevance - when this fails:
  • Situs inversus totalis - complete mirror-image reversal of organ placement (can be asymptomatic)
  • Heterotaxy / Situs ambiguus - randomized organ placement, often with serious cardiac defects
  • Kartagener syndrome (Primary Ciliary Dyskinesia) - defective cilia fail to generate nodal flow, leading to situs inversus + bronchiectasis + infertility
