Stain

Hematoxylin & Eosin (H&E)
  • Eosinophilic (pink/red): colloid inside follicles, RBCs in capillaries, connective tissue

Key Structures Visible

1. Connective Tissue Capsule (top-left)

  • A thin fibrous capsule is seen at the edge of the section
  • Outside it: loose connective tissue and adipose (fat cells appearing as pale empty spaces)

2. Thyroid Follicles (dominant feature)

This is the hallmark architecture of the thyroid. Each follicle is:
  • A round to oval closed sphere of varying sizes
  • Lined by a single layer of follicular epithelial cells (simple cuboidal to low columnar) - their dark round nuclei form a ring around the lumen
  • Filled with colloid - the homogeneous, glassy, pale-to-deep pink material inside each follicle
  • Colloid is thyroglobulin, the iodinated precursor to T3/T4. Its staining intensity reflects the functional state of the gland (pale/washed-out = active gland rapidly secreting; deeply eosinophilic = quiescent/inactive storage state)

3. Interfollicular Stroma

  • Delicate loose connective tissue between follicles
  • Rich capillary network (small round profiles containing bright red RBCs) - reflecting the gland's high vascularity, needed for rapid hormone delivery

4. "Scalloping" (if present at higher power)

  • In an active thyroid, the colloid edge shows peripheral resorption vacuoles (scalloping), where follicular cells are endocytosing thyroglobulin to cleave T3/T4

Physiology Recap (what you're seeing)

The follicular cells synthesize thyroglobulin, store it as colloid, and when stimulated by TSH (from the anterior pituitary), endocytose it back, cleave it into T4 (thyroxine) and smaller amounts of T3 (triiodothyronine), and release them into the bloodstream. T4 is peripherally converted to the more active T3 in target tissues.
