Duphalac lactulose adult dose ml constipation hepatic encephalopathy

Duphalac is a brand of lactulose solution (3.335 g/5 mL). The adult dose depends on the indication:

Duphalac (Lactulose) — Adult Dosing

Constipation

SourceDose
BNF (NICE)15 mL twice daily initially, adjusted to response
NHS15–30 mL per day (may be split into doses)
The dose is titrated up or down depending on stool frequency. The goal is 1–3 soft stools per day.

Hepatic Encephalopathy (Portal-Systemic Encephalopathy)

SourceDose
BNF (NICE)30–50 mL three times daily
NHS30–50 mL three times daily
StatPearls (NIH)15–30 mL two to four times daily
MIMS30–45 mL three to four times daily, adjusted to achieve 2–3 soft stools/day
For hepatic encephalopathy, dosing is guided by stool output — the target is 2–3 semi-soft stools per day.

Rectal (Enema) — Hepatic Encephalopathy (if unable to take orally)

  • 300 mL lactulose in 700 mL water or normal saline, retained for 30–60 minutes every 4–6 hours.

Key point: The dose is always titrated to effect. Start at the lower end and adjust based on the patient's response and stool frequency.
