Cefpodoxime Dose for a 2-Month-Old Child

Important safety note: Cefpodoxime (brand name Vantin) is approved for use from 2 months of age onwards. Safety and efficacy in infants under 2 months have NOT been established (per FDA labeling). For a child at exactly 2 months, use with caution and only under physician supervision.

Dosing (Infants 2 months to 12 years)

The dose is weight-based, given as an oral suspension:

Practical example

A typical 2-month-old weighs approximately 5-6 kg:
  • Dose = 5 mg/kg x 5 kg = 25 mg per dose, twice daily
  • Using 50 mg/5 mL suspension = 2.5 mL every 12 hours

Key points

  • Not for infants < 2 months - safety not established
  • Renal adjustment needed if CrCl < 30 mL/min (extend interval to every 24 hours)
  • Should be taken with food to enhance absorption
  • Cefpodoxime is a 3rd generation oral cephalosporin; consider whether it is the most appropriate choice for the age group - in a 2-month-old with serious infection, IV antibiotics and hospital admission are often more appropriate

Clinical reminder: In a 2-month-old infant with any significant infection, always evaluate for the need for hospitalization and IV antibiotics. Cefpodoxime is an oral drug and may not be appropriate for severe or systemic infections in this very young age group. Always consult current local guidelines and a pediatrician.
