Cold Chain

The cold chain is the system of storing and transporting vaccines (and other temperature-sensitive biologics/pharmaceuticals) at controlled low temperatures, continuously, from the manufacturer to the point of use, so the product retains its potency. In public health/immunization terms it is defined as "a system of storage and transport of vaccines at low temperature from the manufacturer to the actual vaccination site." - Park's Textbook of Preventive and Social Medicine, p. 121

Why it matters

Vaccines are biological products sensitive to heat, freezing, or light. Once potency is lost due to a temperature breach, it cannot be regained - there is no way to test a vaccine's potency visually in the field, so strict adherence to temperature limits is the only safeguard. The system is built around the "6 rights" of supply chain: the right vaccine, in the right quantity, at the right place, at the right time, in the right condition (no temperature break), and at the right cost.

Temperature sensitivity groups

  • Freeze-sensitive vaccines (must be kept above 0°C, protected from sub-zero temps): Hepatitis B, DTaP/DTwP-based combinations, Hib (liquid), HPV, IPV, pentavalent, hexavalent, TT/Td, cholera vaccine.
  • Heat-sensitive vaccines: arranged by textbook (Park's) into six heat-sensitivity categories (Group A = most heat sensitive to Group F = least heat sensitive), e.g. OPV and BCG being more heat-stable when freeze-dried/unopened, while reconstituted freeze-dried vaccines lose potency rapidly after mixing (use within 4 hours or per manufacturer guidance).

Electrical:
  • Walk-in-Freezer (WIF): -15°C to -25°C, installed at national/state/regional stores, used for bulk OPV storage and making frozen ice packs.
  • Walk-in-Cooler (WIC): +2°C to +8°C, for bulk storage of BCG, Hep B, DPT, pentavalent, IPV, measles, TT at state/regional/some district stores.

Non-electrical/transport equipment: cold boxes, vaccine carriers, and ice packs - used to move vaccines the "last mile" to session sites, where the risk of a cold chain break is greatest (sub-centre/village level), which is why vaccines are not stored there and must be resupplied via the cold chain for each session.

Key operational practices

  • Continuous temperature monitoring with recorders/alarms on WIC/WIF units.
  • A designated person at each health facility with overall responsibility for cold chain management (with a trained backup).
  • Open Vial Policy: unused opened multi-dose vials must be returned to the Cold Chain Point at the end of a session and segregated (reusable vs. discard) based on vaccine type and time since opening.
  • Twice-daily temperature logging (per CDC guidance), records retained for years, and immediate corrective action for any excursion outside range.

Current/global guidance (supplementing the textbook)
