Based on the clinical picture — face puffiness (angioedema) + itchy skin after eating fish — this is an acute allergic (IgE-mediated) reaction to fish. Here is the treatment approach:

Diagnosis: Acute Food Allergy (Fish) — Urticaria + Angioedema

Symptoms explained:
  • Face puffiness = Angioedema (swelling of deeper skin layers)
  • Itchy skin = Urticaria (hives)
  • Trigger = Fish (a common IgE-mediated food allergen, releases histamine from mast cells)

Drug Treatment (Step-by-Step)

🟢 MILD–MODERATE (Itching + Face Swelling, No throat/breathing involvement)

First-line: H1 Antihistamines
DrugDose (Adult)Notes
Cetirizine (Zyrtec)10 mg once dailyPreferred — second-generation, non-sedating
Loratadine (Claritin)10 mg once dailyNon-sedating, safe in pregnancy
Fexofenadine (Allegra)120–180 mg once dailyNon-sedating
Chlorphenamine (first-gen)4 mg 3× dailySedating — use if above not available
Diphenhydramine (Benadryl)25–50 mg every 6 hrsSedating; fast-acting
Second-generation antihistamines (cetirizine, loratadine) are now the first-line choice. First-generation (chlorphenamine, diphenhydramine) are sedating and no longer preferred, but still effective if needed.

🟡 MODERATE–SEVERE (Significant swelling, multiple body areas)

Add a short course of oral corticosteroid:
  • Prednisolone 30–50 mg/day for 3–5 days (adult)
  • Reduces inflammation and prevents biphasic reactions

🔴 SEVERE (Throat swelling, difficulty breathing, low BP, anaphylaxis signs)

Epinephrine (Adrenaline) — IMMEDIATE PRIORITY
  • Epinephrine 0.3–0.5 mg IM (intramuscular, outer thigh)
  • This is the treatment of choice for anaphylaxis or severe angioedema of the throat
  • Call emergency services immediately

Key Points

  • Watch for danger signs: throat tightness, voice change, difficulty swallowing/breathing, dizziness → these indicate anaphylaxis → give epinephrine immediately
  • Fish allergy can worsen with repeat episodes — the next reaction may be more severe
  • The patient should avoid all fish and fish-containing foods going forward
  • Refer to an allergist for allergy testing (skin prick test or specific IgE) after the acute episode resolves

⚠️ Clinical note: If facial swelling is progressing rapidly or involves the lips/tongue/throat, treat as anaphylaxis with epinephrine — do not wait.
