Ademetionine (SAMe)
Ademetionine is the pharmaceutical form of S-adenosyl-L-methionine (SAMe), a naturally occurring molecule made from methionine + ATP. It is an important methyl-group donor in the body. Harper's Illustrated Biochemistry describes SAMe as the principal source of methyl groups and notes its synthesis by methionine adenosyltransferase (p. 320).
Mechanism of action
SAMe has three major biochemical roles:

Methylation
- Donates methyl groups to DNA, proteins, phospholipids, and neurotransmitter-related pathways.
- This may alter cell-membrane phospholipids and influence synthesis or metabolism of serotonin, dopamine, and norepinephrine.
- These effects provide the proposed rationale for its antidepressant activity.

Transsulfuration and antioxidant support
- SAMe metabolism contributes to production of cysteine and glutathione, an endogenous antioxidant.
- In liver disease, this may support hepatocyte defense against oxidative injury.

Aminopropylation
- Decarboxylated SAMe supplies groups for synthesis of the polyamines spermidine and spermine, which participate in cellular growth and membrane stability. Harper's Illustrated Biochemistry, p. 320.
In cholestatic liver disease, ademetionine is thought to improve hepatocyte membrane fluidity and bile formation/transport. Its clinical benefit depends on the indication and formulation.
Uses
Use varies by country. In some places it is prescribed; in the United States, SAMe is generally sold as a dietary supplement rather than an FDA-approved prescription drug.

Intrahepatic cholestasis / chronic liver disease
- Used in some countries as supportive therapy for pruritus and biochemical cholestasis.
- It does not replace evaluation and treatment of the cause of cholestasis, such as obstruction, viral hepatitis, drug injury, or pregnancy-related disease.
- A 2024 systematic review found that the liver-health evidence is heterogeneous and that stronger trials are still needed: recent systematic review.

Depression
- Sometimes used as monotherapy or, more often, an adjunct to a conventional antidepressant in selected patients.

Osteoarthritis
- Used in some settings for pain and functional symptoms. Evidence is mixed and it is not routinely preferred over established treatments.

Other off-label or supplement uses
- Fibromyalgia and other chronic pain conditions have been studied, but evidence is limited or inconsistent.
Adverse effects
Most are mild and dose-related:

Important precautions and interactions
- Bipolar disorder: Avoid unless managed by a psychiatrist. SAMe can trigger mania or hypomania.
- Serotonergic drugs: Use caution with SSRIs, SNRIs, MAO inhibitors, tricyclic antidepressants, tramadol, linezolid, triptans, dextromethorphan, or St John's wort. Combining these may increase the risk of serotonin toxicity.
- Seek urgent care for agitation, confusion, fever, sweating, diarrhea, tremor, muscle rigidity, fast heartbeat, or severe blood-pressure changes, especially after combining it with antidepressants.
- Pregnancy or breastfeeding: use only under clinician guidance. This is particularly important in suspected intrahepatic cholestasis of pregnancy.
- Do not self-treat jaundice, dark urine, pale stools, severe itch, fever, or right-upper abdominal pain with SAMe. These require medical assessment.
Bottom line
Ademetionine is a metabolic methyl donor with plausible effects on neurotransmitter pathways, liver antioxidant capacity, and bile transport. It is used mainly for cholestatic liver conditions and sometimes for depression or osteoarthritis, but the strength of evidence and regulatory status vary. The main safety concerns are insomnia/anxiety, drug interactions with antidepressants, and induction of mania in people with bipolar disorder.
