Ondansetron Pediatric Dosing

General Weight-Based Dose

• 0.1-0.15 mg/kg/dose IV or PO
• Maximum single dose: 8 mg (some references allow up to 16 mg IV in older children/adolescents)
• Recommended for children ≥6 months of age

By Indication

1. Postoperative Nausea & Vomiting (PONV)

2. Acute Gastroenteritis

3. Chemotherapy-Induced Nausea & Vomiting (CINV)

• 0.1-0.2 mg/kg/dose IV or PO, once pre-therapy then every 8 hours
• Maximum: 8 mg/dose
• Oral dosing by age:
  • 4-11 years: 4 mg PO 30 min before chemotherapy, then every 4 hours x 2 doses, then every 8 hours x 1-2 days
  • ≥12 years: 8 mg PO (adult dosing applies)

4. General Nausea/Vomiting

• 0.1-0.15 mg/kg/dose IV or PO every 8 hours PRN
• Maximum: 8 mg/dose

• Injection: 2 mg/mL
• Oral solution: 0.8 mg/mL (4 mg/5 mL)
• Tablets: 4 mg, 8 mg
• Orally disintegrating tablets (ODT): 4 mg

Key Safety Notes

• Risk of QTc prolongation - avoid in patients with congenital long QT syndrome or those on other QT-prolonging drugs
• Serotonin syndrome risk when combined with other serotonergic agents
• Clearance is higher per body weight in young children, so weight-based dosing is important
• Maximum IV single dose: 16 mg (due to QTc risk) - though 8 mg/dose max is used in most pediatric protocols
• IV infusion: administer over 2-5 minutes (not as a rapid bolus)
