This is Quetiapine Fumarate 12.5 mg, a second-generation (atypical) antipsychotic from Trianga Pharma. The 12.5 mg dose is the lowest available - typically used as a starting dose or for sensitive populations.

Common Off-Label Uses

  • Insomnia / sleep disorders - at very low doses (12.5-25 mg), its sedative H1-blocking properties are used; this is actually one of the most common reasons for the 12.5 mg dose specifically
  • Generalized anxiety disorder (GAD)
  • PTSD - adjunctive treatment
  • Agitation / behavioral disturbances in dementia (though not recommended due to black box warning)
  • Borderline personality disorder - as an adjunct
Why 12.5 mg specifically? This ultra-low dose is mostly used for insomnia or anxiety (not psychosis), exploiting quetiapine's strong antihistamine (H1) sedation at low doses, rather than its antipsychotic dopamine-blocking effects which require much higher doses (300-800 mg/day).

Mechanism of Action

Quetiapine blocks multiple receptors:
  • D2 dopamine receptors - antipsychotic effect (fast dissociation = fewer extrapyramidal side effects)
  • 5-HT2A serotonin receptors - reduces EPS, improves negative symptoms and mood
  • H1 histamine receptors - sedation (dominant at low doses like 12.5 mg)
  • Alpha-1 adrenergic receptors - sedation, orthostatic hypotension
  • NET (norepinephrine transporter) - via active metabolite norquetiapine, contributes to antidepressant effects

Key Warnings

  • Black box warning: Increased mortality in elderly patients with dementia-related psychosis - NOT approved for this use
  • Black box warning: Increased risk of suicidal thoughts in children/adolescents/young adults
  • Monitor for: metabolic syndrome (weight gain, hyperglycemia), QT prolongation, tardive dyskinesia, orthostatic hypotension, cataracts (with long-term use)

This is a prescription-only (Rx) medication. It should only be used under a doctor's supervision. The 12.5 mg dose is far below the therapeutic antipsychotic range and is almost always prescribed for sleep or anxiety rather than for psychosis.
