PART 1: GENITAL WARTS (Condyloma Acuminatum)

What is it?

A neoplastic (tumor-forming) lesion of the penis
Also called Condyloma acuminatum
Looks like a cauliflower - bumpy, fleshy growth on the genitals
These are warts caused by a virus (HPV)

What causes it? - Risk Factor

HPV types 6 and 11 are responsible
HPV = Human Papillomavirus - a common sexually transmitted virus

How does HPV cause warts? - Pathogenesis (Step by Step)

Step 1: HPV enters cells and produces two proteins: E6 and E7

Step 2:

E6 attacks and inactivates P53 (a tumor suppressor gene - think of it as a "brake" that stops cells from growing out of control)
E7 attacks and inactivates Rb (another tumor suppressor gene - another "brake")

Step 3: With both brakes destroyed, cells start multiplying uncontrollably

Step 4: This leads to Warts first, and if unchecked, can lead to Cancer

Simple analogy: Imagine your car's brakes (P53 and Rb) being cut - the car (cell) just keeps going and going, eventually crashing (becoming cancerous).

What do they look like? - Clinical Appearance

Red, velvety, bumpy (excrescent) lesions
Most commonly found on the glans (tip of penis) and inner surface of the prepuce (foreskin)

What do they look like under the microscope? - HPE (Histopathology)

Acanthosis - The skin (epithelium) becomes thickened
Hyperkeratosis - The top layer of the epithelium becomes abnormally thick and hardened
Branching, villous pattern - The tissue forms finger-like branches, like coral or a tree branch

Right image (High power view) - Koilocytes: These are the signature cells of HPV infection. They have 3 special features:

Large polyhedral cells - Bigger than normal cells, multi-sided shape
Raisin-like nucleus - The cell's nucleus is shrunken, dark, and wrinkled - looks like a raisin
Perinuclear halo - There is a clear empty space (halo) around the nucleus - this is the most important identifying feature of HPV infection

Simple memory trick: Koilocyte = Koilocyte has a Koronavirus-like halo around the nucleus. See the halo = think HPV.

PART 2: PRE-MALIGNANT LESIONS OF THE PENIS

These are lesions that are NOT yet cancer but have the potential to become cancer. There are 3 types:

1. Bowen's Disease

Solitary	Single lesion (just one spot)
Elderly population	Seen in older men
10% risk of Ca	Has a 10% chance of turning into cancer - moderate risk

2. Bowenoid Papulosis

Feature	Simple Explanation
Multiple papules	Multiple small bumps (not just one)
Young, sexually active population	Seen in young people who are sexually active
Low risk of Ca	Very low chance of becoming cancer
Regresses spontaneously	Goes away on its own without treatment - the most important feature!

3. Erythroplasia of Queyrat

Feature	Simple Explanation
Red papules on penis	Red-colored bumps on the penis
Premalignant lesion	It IS a premalignant lesion - can turn into cancer
