1. Inguinal Hernia

The most common hernia, accounting for ~75% of all hernias. Far more common in males.

Indirect (Oblique) Inguinal Hernia

  • Comprises >80% of inguinal hernias
  • The hernia passes through the deep inguinal ring, which lies lateral to the inferior epigastric vessels
  • Follows the inguinal canal obliquely, can descend all the way into the scrotum along the processus vaginalis
  • Two subtypes:

2. Femoral Hernia

Emergency Management (ED)

  4. Strangulated → surgical emergency; do not attempt reduction — risk of reducing gangrenous bowel

Surgical Options

Herniotomy

  • Children with indirect hernia: simply excise and ligate the sac (high ligation at deep ring)
  • In adults: herniotomy alone has high recurrence — herniorrhaphy required

Open Suture Repair

  • Bassini repair (1890): approximates conjoint tendon to inguinal ligament — the foundation of hernia surgery for 100 years
