Definition Myasthenia gravis (MG) is an autoimmune disorder characterized by circulating antibodies directed against postsynaptic nicotinic acetylcholine receptors (nAChRs) at the neuromuscular junction (NMJ) of skeletal muscle. The name literally means "grave muscular weakness."
Anatomical Basis The NMJ is a specialized synapse between a lower motor neuron axon terminal and the motor end plate of a skeletal muscle fibre. Normally:
  • The axon terminal releases acetylcholine (ACh) into the synaptic cleft.
  • ACh binds to nicotinic receptors on the postsynaptic membrane (motor end plate), triggering depolarization and muscle contraction.
In MG, autoantibodies (anti-AChR antibodies, positive in ~85% of generalized cases) block and destroy these receptors, reducing the effective receptor pool at the end plate. Additional antibodies against MuSK (muscle-specific receptor tyrosine kinase), LRP4, and agrin have also been identified.
