CT angiogram abdominal aorta renal artery 3D reconstruction segmentation

What I'm looking at

This is a single axial slice from a contrast-enhanced abdominal CT, photographed off a viewing monitor (note the glare and the "A 227" / "P 185" table-position or window markers). It's taken at roughly the level of the renal hila/lower kidneys, with the vertebral body and paired psoas major muscles seen posteriorly.
Key structures and findings:
  1. Central vascular structure (bright white with pink overlay) – Anterior to the vertebral body sits a densely enhancing structure with two smaller round branches coming off it, all highlighted in a pink/magenta color overlay with a small crosshair (measurement caliper) marker. This pattern is classic for computer-segmented vascular anatomy from a CT angiogram – the main trunk is almost certainly the abdominal aorta, and the two branching round structures are the bilateral renal arteries taking off from it. The color overlay and caliper point indicate this image comes from 3D vascular planning/segmentation software, typically used to:
    • Map renal artery anatomy before living-donor nephrectomy or renal transplantation
    • Evaluate for a renal artery aneurysm or stenosis
    • Plan aortic intervention (e.g., EVAR) where renal artery origins need to be defined relative to an aneurysm neck
    Renal artery aneurysms themselves are uncommon and usually asymptomatic, but carry risk of rupture and can cause renovascular hypertension - CT angiography with 3D reconstruction is the standard way to characterize them (Smith and Tanagho's General Urology, 19th Edition).
  2. Right kidney (image left side) – Shows multiple rounded, low-attenuation (dark) structures clustered together. This could represent either dilated/clubbed calyces (hydronephrosis), multiple simple renal cysts, or a complex cystic kidney - I can't distinguish definitively from this single photographed slice. This would need correlation with non-contrast and delayed/excretory phase images to tell dilated collecting system from cysts.
  3. Left kidney (image right side) – Appears more homogeneous and rounded, without the same cystic/dilated appearance - looks relatively unremarkable on this slice.
  4. Bowel loops anterolaterally on both sides, appearing unremarkable, with visible mesenteric vessels on the left.
  5. Paraspinal (psoas) muscles are symmetric bilaterally - no asymmetry suggesting hematoma or mass.

Clinical interpretation

Put together, this looks like a CT angiogram/vascular-mapping study focused on the aorta and renal arteries, most likely done for renal artery evaluation (aneurysm, stenosis, or pre-transplant/pre-surgical vascular mapping), with an incidental finding of a possibly abnormal-appearing right kidney (cystic vs. dilated collecting system) that would warrant a closer look at the rest of the series.
Important caveats:
  • This is one slice from a photograph of a screen, not the original DICOM series, so window/level, other slices, and any accompanying report are not available to me.
  • I cannot confirm laterality conventions used by the specific scanner/software, measurement values, or whether the pink overlay is annotated with a size that would indicate an aneurysm.
  • A definitive read requires review of the full multiplanar series (axial, coronal, sagittal), the non-contrast phase for calcification, and the radiologist's report with clinical history (e.g., hematuria, hypertension workup, transplant donor evaluation, flank pain).
