Source / Origin

The oesophagus develops from the foregut, the portion immediately caudal to the pharynx. The foregut is lined by endoderm, which gives rise to the epithelium and glands of the oesophagus. The surrounding splanchnic mesoderm contributes the muscular and connective tissue components.

Separation from the Respiratory Tract (Tracheoesophageal Septum Formation)

At approximately week 4, a ventral outgrowth from the foregut called the laryngotracheal (respiratory) diverticulum appears. The foregut then becomes partitioned by two longitudinal ridges - the tracheoesophageal ridges - that grow towards each other from either side.
These ridges fuse in a craniocaudal direction to form the tracheoesophageal septum, which divides the foregut into:
  • Dorsal part - the oesophagus
  • Ventral part - the laryngotracheal tube (future trachea)
The respiratory primordium retains its connection with the pharynx through the laryngeal orifice.

  • Cause: posterior deviation of the tracheoesophageal septum leading to incomplete separation
  • Features: polyhydramnios (fetus cannot swallow amniotic fluid), excessive drooling, regurgitation on feeding
  • Diagnosis confirmed by failure to pass a nasogastric tube into the stomach

2. Oesophageal Stenosis

  • Narrowing, usually in the distal third
  • May present as a web or ring

3. Tracheoesophageal Fistula (TEF)

  • Most common type (88%): upper oesophagus ends in blind pouch + lower segment fistulates with trachea
  • H-type (no atresia): 4% of cases
