Your question was just "insulin injection," so this is a general overview. If you want a specific angle, such as technique, dosing, types, or complications, tell me and I'll go deeper.

Route and why it matters

  • Long-term insulin treatment relies mostly on subcutaneous injection. It gives near-normal glycemia but differs from physiology in two ways. Absorption doesn't match the fast rise and fall of endogenous insulin, and insulin goes into the peripheral circulation instead of the portal circulation. - Goodman & Gilman's The Pharmacological Basis of Therapeutics, p. 1049-1050
  • Regular U-100 insulin can also be given IV or IM in monitored settings. U-500 regular insulin is for subcutaneous use only. - Goodman & Gilman's, p. 1051

Main insulin types and timing

TypeExamplesTiming and notes
Short-acting (regular)Regular human insulinInject 30-45 min before a meal. It forms hexamers, which slows absorption.
Rapid-acting analoguesLispro, aspart, glulisineInject 15 min or less before a meal. They dissociate quickly into monomers. They cause less hypoglycemia than regular insulin.
IntermediateNPHCloudy suspension with zinc and protamine. Given at bedtime or twice daily.
Long-acting analoguesGlargine, detemir, degludecBasal coverage. Glargine is acidic (pH 4), so it cannot be mixed with short-acting insulins.

Dosing basics

  • In type 1 diabetes, the total daily dose is usually 0.4-0.7 units/kg/day.
  • Obese patients, patients with type 2 diabetes, and pubertal adolescents may need about 1-2 units/kg/day because of insulin resistance.
  • Basal insulin is about 40-50% of the total daily dose. The rest is mealtime insulin, plus a correction dose.
  • A single daily dose of long-acting insulin alone is not enough for optimal control in most patients.
  • Source: Goodman & Gilman's, p. 1052

Injection technique

  • Site: The abdomen is the preferred site in the morning because insulin is absorbed 20-30% faster there than from the arm. Other sites are the thigh, buttock, and upper arm. - Goodman & Gilman's, p. 1052
  • Absorption speeds up with heat, massage, exercise, and injecting into muscle. Depth, volume, concentration, smoking, and local blood flow also affect it. - Goodman & Gilman's, p. 1052
  • Rotation: Rotate sites to avoid scarring, lipohypertrophy, and lipoatrophy. The ADA 2019 Standards recommend rotation for this reason. An older ADA statement advises rotating within one body area, for example systematically around the abdomen, rather than moving between areas. Moving between areas changes absorption speed.
  • Needle length: Short pen needles (4-6 mm) work well and are well tolerated, including in obese adults. Needles of 8 mm or longer should be avoided. The ADA review notes that unintended IM injection is more likely in lean or young patients, in limb sites, and with longer needles. Short needles are usually inserted at 90 degrees.
  • Pens and needles: Use a new needle for every injection. A pen is single-patient use only and must never be shared, because blood can be present in it. CDC injection safety guidance covers this.
  • Variability: Time to peak effect can vary by about 50% between people, and even between doses in the same person. - Goodman & Gilman's, p. 1051

Complications

  • Hypoglycemia is the major risk. Weight gain is common but modest.
  • Lipohypertrophy is raised, rubbery fat deposits from repeated injections at one site. It is attributed to the lipogenic action of high local insulin concentrations. It can cause erratic absorption, and patients should avoid injecting into it. - Goodman & Gilman's, p. 1052; ROSEN's Emergency Medicine (Cutaneous Manifestations)
  • Lipoatrophy is now rare with human and analogue insulins.
  • Allergy is uncommon. It can come from aggregated insulin, contaminants, or additives such as protamine or zinc. - Goodman & Gilman's, p. 1052

Recent evidence

A quick PubMed check of the last 2 years found no meta-analysis that contradicts the points above. Two recent meta-analyses look at subcutaneous insulin in diabetic ketoacidosis. One covers subcutaneous versus IV infusion (PMID 39090718). The other covers early subcutaneous basal insulin added to IV infusion (PMID 41208563). These are DKA-specific and don't change routine outpatient technique.
This is educational information. Doses and regimens should be set by the treating clinician. Which part would you like in detail: technique teaching, regimen design, or lipohypertrophy and other complications?

Insulin Injection Technique Patient Handout

PDF Document · PDF

Page 1: Getting ready and injecting
  • Blank lines for the patient's name, the date, and their insulin(s).
  • A pre-injection checklist: wash hands, check the label and expiry, and look at the insulin. It covers how to mix cloudy insulin such as NPH.
  • Seven numbered steps: attach a new needle, prime with 2 units, dial the dose, pick a site, insert at 90 degrees, press and count to 10, then remove the needle and dispose of it in a sharps container.
  • A tips box on timing. Rapid-acting insulin is usually taken within 15 minutes of eating, and regular insulin 30 to 45 minutes before a meal.
Page 2: Sites, needles and storage
  • A front and back body diagram with the four injection areas highlighted: abdomen, thighs, buttocks and back of the upper arms. Each has a short note on absorption speed and practical tips.
  • Rotation advice: use one body area for the same time of day, move about 1 inch from the last spot, avoid lumps, and check sites regularly.
  • A needle table covering short 4 to 6 mm needles, one new needle per injection, and never sharing pens.
  • Storage: unopened insulin in the fridge, and an in-use pen at room temperature, usually up to 28 days (check the brand).
Page 3: Problems and personal plan
  • A low blood sugar box with the signs, the 15-15 rule, and when to call 911.
  • A troubleshooting table for leaking, missed doses, a stuck pen, site reactions and sick days.
  • A blank table for the care team to enter insulin, dose, timing and notes.
  • A "call your care team if" box with blank phone number lines.
Before you hand it out
  • Make sure the sick-day, missed-dose and low-sugar advice matches your own protocols. Those parts are kept general on purpose, and the plan table is meant for the clinician to fill in.
  • It covers insulin pens only. Vial-and-syringe technique, pumps and U-500 pens would need a separate version.
  • The body diagram is a simple drawing I made, not a medical illustration. Swap in your clinic's own figure if you prefer.
  • Three sources feed the content: Goodman & Gilman's for insulin types, absorption and rotation, the ADA Standards for short needles and technique, and CDC guidance for single-patient pen use. The 15-15 rule, priming step and 10-second count are standard teaching practice, not taken from those sources.
I can make a larger-print version, a Spanish version, or a vial-and-syringe version if you'd like one.

Insulin Injection Technique Handout LARGE PRINT

PDF Document · PDF

Your message ended with "0 years", which I took as a stray typo and ignored. If you meant something specific, such as a version for older adults, tell me.
What changed from the standard version
  • Text size: Body text is 16 pt with generous line spacing, up from 10.5 pt. Headings are 22 pt and the title is 30 pt. All text is black or dark navy on white or pale backgrounds for high contrast.
  • Layout: It runs 9 pages with one idea per page, so nothing is crowded. The pages are:
    1. Title, name fields, and the pre-injection checklist
    2. Steps 1 to 4
    3. Steps 5 to 7, plus a tips box
    4. Enlarged body diagram with a large-text legend
    5. Site rotation
    6. Needles and storage
    7. Low blood sugar (the 15-15 rule, with a separate "Call 911" box)
    8. Troubleshooting table
    9. Blank insulin plan table, "call your care team" box, and phone lines
  • Wording: Sentences are shorter and use plain words. Step numbers are in larger boxes. The insulin plan table has taller rows so handwriting fits.
Content differences
  • I dropped the "Notes" column from the plan table so the remaining columns could be wider.
  • The clinical content is otherwise the same as the standard version.
Some pages have empty space at the bottom. That is intentional, so you can print it as is or staple it as a booklet.
For people with significant vision loss, a 20 pt or larger version, or a Spanish version, can be made on request.
