Gordon's Classification of Death

Published: Professor I. Gordon, 9 September 1944, in the British Medical Journal. It was a landmark paper in forensic pathology that replaced the vague and scientifically inadequate term "asphyxia" (literally "without a pulse") with the more precise term anoxia ("without oxygen").

Core Concept

Gordon postulated that all vital functions of the body depend upon the availability and utilization of oxygen by tissues. Tissue anoxia ultimately leads to cardiac failure and death. His classification lays stress on the pathogenesis of different forms of death of medicolegal importance.

The Four Types

1. Anoxic Anoxia

Defective oxygenation of blood in the lungs
Oxygen fails to reach the blood. Caused by:
  • Obstruction to the air passage: suffocation, smothering, overlaying
  • Obstruction to passage of air down the respiratory tract: drowning, choking, throttling, hanging, strangulation
  • Excessive compression of the chest and abdominal wall
  • Primary cessation of respiratory movements (respiratory failure): narcotic poisoning, electrical injury
  • Inhalation of CO2 and carbon monoxide gases

2. Anaemic Anoxia

Reduced oxygen-carrying capacity of the blood
Blood reaches the lungs but cannot carry enough oxygen. Caused by:
  • Acute poisoning by CO (carboxyhemoglobin formation)
  • Chlorates
  • Nitrites

3. Histotoxic Anoxia

Depression of the oxidative process in the tissues
Oxygen is delivered to the tissues but the cells cannot utilize it. Caused by:
  • Hydrocyanic acid (cyanide) poisoning - inhibits cytochrome oxidase

4. Stagnant Anoxia

Insufficient circulation of blood in the tissues
Blood carries adequate oxygen but does not circulate adequately to deliver it. Caused by:
  • Traumatic shock
  • Heat stroke
  • Acute irritant and corrosive poisoning
