1. Essay Pattern (2 essays per paper, 15 marks each)
  • One essay is ALWAYS a clinical case scenario (patient with symptoms, lab values given - diagnose + discuss)
  • Second essay is usually a direct topic essay (sometimes also clinical)
  • Essay topics rotate across: Metabolic/Endocrine - Renal - Haematology - Infections
  • FUO/PUO appeared as essay TWICE in a row (Apr 2024 and Dec 2024) - rare but notable
2. Short Note Patterns
  • Ethics question ALWAYS present at Q12 position - this is a fixed slot
  • Poisoning is almost always present (Paracetamol 2x, OP poisoning 1x)
  • DIC appears in nearly every paper
  • Haematology (one topic per paper) is consistent
  • Nephrology (one short note per paper) is consistent
3. Question Style Shift
  • Moving from direct-definition questions toward clinical case-based essays (trend since 2023)
  • Short notes remain straightforward but occasionally case-linked (pain, fever in older adults)
  • Pharmacology sneaks in as short notes (thyroxine, antithyroid, insulin analogues, opioids, immunosuppressants)

STEP 6: COMMONLY IGNORED BUT HIGH-VALUE AREAS

These topics are asked but students often skip them:
  1. Medico-legal/Ethics - Q12 is FIXED. Always a 5-mark short note. Easy marks if prepared. Topics rotate: breach of confidence, organ donation, physician as communicator, informed consent, euthanasia, medical negligence.
  2. Pharmacology-embedded short notes - Thyroxine pharmacology, insulin analogues, opioid analgesics, immunosuppressants were all asked. Examiner uses short notes to test pharmacology without it being a separate paper.
  3. Pain management - Asked in 2 consecutive papers in different forms (neuropathic pain in Dec 2023, pain assessment in April 2024). Analgesia ladder, WHO pain ladder, opioid risks.
  4. Clinical differentials in diarrhoea - Small vs large bowel asked. Upper GI vs lower GI bleeding pattern likely next.
  5. Frostbite / Environmental medicine - Seemingly obscure but appeared. Don't ignore: heat stroke, drowning, electric shock, altitude sickness.
  6. Probiotics / Newer therapeutics - Shows examiner interest in current clinical practice, not just classical medicine.
