Levocetirizine in a 7-Month-Old with Scabies

Is levocetirizine allowed at 7 months?

Yes — levocetirizine is approved and safe from 6 months of age.
Per FDA-approved labeling (Xyzal oral solution), levocetirizine is indicated from 6 months of age for chronic idiopathic urticaria and perennial allergic rhinitis. A 7-month-old falls within the approved age range.
Dose for 6 months – 5 years (oral solution):
  • 1.25 mg (2.5 mL of 0.5 mg/mL solution) once daily in the evening
  • Do not exceed 1.25 mg/day in this age group
Important caveat: Avoid in infants with any renal impairment — children 6 months to 11 years with kidney disease should NOT receive levocetirizine (impaired clearance leads to drug accumulation).

Role of antihistamines in scabies

One important point: scabies pruritus is primarily non-histaminergic (mediated by immune sensitization to mite antigens, not histamine release). Antihistamines like levocetirizine provide symptomatic relief of itch but do not treat the infestation. The itch can also persist for 2–4 weeks after successful treatment due to residual immune hypersensitivity — antihistamines are useful during this post-treatment phase.

Scabies Treatment in a 7-Month-Old (the Priority)

The antiparasitic treatment must be given first. For this age group:
TreatmentStatus for infants
Permethrin 5% creamFirst-line — FDA-approved ≥2 months of age; apply head-to-toe (including scalp/face in infants), leave 8–12 hrs, repeat on day 8
Sulfur 5–10% in petrolatum✅ Considered safe in infants; used where permethrin unavailable; 3 consecutive overnight applications
Ivermectin (oral)⚠️ Not well-established <15 kg; recent small series suggest safety but traditionally not first-line under 15 kg
Lindane❌ Contraindicated in infants — CNS toxicity risk
For crusted (Norwegian) scabies (which "severe" scabies may indicate), combination of permethrin + oral ivermectin is often required, along with a keratolytic (e.g. salicylic acid) to reduce hyperkeratosis.

Summary

  • Levocetirizine: Yes, allowed at 7 months — 1.25 mg once daily in the evening (oral solution). Provides symptomatic itch relief.
  • Primary treatment: Permethrin 5% cream — apply to full body including scalp and face, days 1 and 8.
  • Treat all household contacts simultaneously to prevent reinfestation.
  • Wash all clothing/bedding used in the past 3 days in hot water.
