NHS coughing up blood hemoptysis when to seek urgent medical help

Brown sputum can sometimes be mucus mixed with a small amount of old blood, but it can also occur with throat irritation, a chest infection, smoking/vaping, or heavy air pollution. In Delhi, pollution can worsen cough and airway irritation, but do not assume pollution is the cause.
Because this is new and could represent blood in sputum, arrange an urgent in-person medical assessment today or within 24 hours, especially if it happens again. A clinician may check your lungs and decide whether you need a chest X-ray or sputum tests. Tuberculosis is one possible cause of coughing blood in India, particularly if you have cough lasting over 2 weeks, fever, night sweats, weight loss, or TB exposure, but one episode does not mean you have TB.
Go to an emergency department now / call 112 in India if you have:
  • More than a few streaks or repeated amounts of blood
  • Trouble breathing, chest pain, fainting, fast heartbeat
  • High fever, severe weakness, or coughing up clots
  • Recent long travel, leg swelling/pain, or a history of blood clots
Until seen: avoid smoking/vaping, do not take aspirin or ibuprofen unless prescribed, stay hydrated, and take a photo of the sputum if it occurs again. Coughing up blood, even small amounts, should be checked, as NHS guidance advises.
Was it definitely from coughing out of your chest, rather than bleeding gums, a nosebleed dripping backward, or vomit? And do you have cough, fever, chest pain, shortness of breath, weight loss, or smoking/vaping history?

coughing up blood emergency symptoms seek emergency care official health guidance

Go to the emergency department now or call 112/108 in Delhi if brown sputum is accompanied by any of these:
  • More than a few streaks/spots of blood, repeated bloody sputum, clots, or bleeding that does not stop
  • Shortness of breath, wheezing that is severe, blue lips, or difficulty speaking full sentences
  • Chest pain, especially sudden sharp pain that worsens with breathing, or pain in the upper back
  • Very fast heartbeat, fainting, severe dizziness, confusion, marked weakness, or cold/clammy skin
  • Sudden breathlessness after travel/immobility, or with one-sided leg swelling/pain, since this can signal a lung blood clot
  • High fever with severe illness, confusion, or rapidly worsening cough
  • You take a blood thinner such as warfarin, apixaban, rivaroxaban, dabigatran, or have a bleeding disorder
  • Known serious lung disease, tuberculosis, cancer, or recent chest injury/procedure
The combination of coughing blood with breathing difficulty, chest/upper-back pain, or a fast heartbeat needs emergency assessment, as described in NHS emergency guidance.
If it was only one small brown streak, you feel otherwise well, and it does not recur, it is less likely to be an immediate emergency, but you should still arrange a doctor visit within 24 hours, particularly in Delhi where TB and respiratory infections need consideration. If you get a cough lasting over 2 weeks, fever, night sweats, weight loss, or the brown sputum returns, seek prompt evaluation.

blood in sputum when to see doctor health guidance brown sputum official

If you have only brown sputum and no breathlessness, chest pain, fever, dizziness, fast heartbeat, or visible red blood, it is less likely to be an immediate emergency.
Brown sputum may be from:
  • A very small amount of old blood from irritated nose, throat, or airways
  • Dust/smoke/pollution exposure
  • Smoking or vaping
  • A mild respiratory infection, even before other symptoms develop
However, because this is new and brown sputum can sometimes contain old blood, please arrange a visit with a general physician or chest specialist within the next 24-48 hours, especially if it occurs again tomorrow. They can examine you and decide if you need a chest X-ray or sputum testing. Medical sources advise evaluation for any possible blood in sputum, even when the amount is small, because the cause needs to be identified. MedlinePlus explains that brown sputum can represent old blood or inhaled dust/material.
For tonight:
  • Drink water or warm fluids.
  • Avoid smoking, vaping, alcohol, and dusty/polluted outdoor air. Use a well-fitting N95 mask outdoors in Delhi.
  • Do not take aspirin or ibuprofen unless a clinician has advised it.
  • Note whether it is truly coughed up from the chest, versus mucus from the nose/throat.
  • Take a clear photo if it happens again, as it may help the doctor.
Seek urgent care immediately if it becomes red/bloody, increases in amount, keeps recurring, or you develop cough, fever, night sweats, weight loss, chest pain, or breathlessness. In Delhi, a persistent cough or blood-stained sputum also warrants checking for respiratory infection including TB.
