Main pattern in the report

This is a hepatocellular liver-enzyme elevation:
  • ALT/SGPT: 121 U/L (about 2.7 times upper limit)
  • AST/SGOT: 63.9 U/L (about 1.8 times upper limit)
  • GGT: 65.9 U/L (mildly high)
  • Bilirubin, alkaline phosphatase, albumin, creatinine, and eGFR are normal.
There is also mixed dyslipidemia:
  • LDL cholesterol: 152 mg/dL - high
  • Triglycerides: 185 mg/dL - borderline high
  • HDL cholesterol: 39 mg/dL - low
  • Total cholesterol: 222 mg/dL - high
  • Non-HDL cholesterol: 183 mg/dL - high
The AST:ALT ratio is 0.53, meaning ALT predominates. This pattern is more consistent with metabolic fatty liver or other non-alcohol-related hepatocellular injury than with classic alcohol-associated hepatitis, where AST usually predominates and AST:ALT is commonly >2. Mildly raised uric acid, 8.3 mg/dL, often travels with insulin resistance and metabolic risk.

Most probable cause

Metabolic dysfunction-associated steatotic liver disease (MASLD), previously called NAFLD

This is the leading possibility, potentially with steatohepatitis (MASH), given the combination of:
  1. ALT-predominant transaminitis
  2. High LDL and triglycerides with low HDL
  3. Mild GGT elevation
  4. Preserved bilirubin, albumin, alkaline phosphatase, and renal function
  5. Mild hyperuricemia
MASLD is associated with excess liver fat and metabolic factors such as central obesity, prediabetes or diabetes, hypertension, and dyslipidemia. It cannot be confirmed by blood tests alone. Liver ultrasound and metabolic assessment are appropriate next steps. A recent AASLD update also describes MASH as linked to obesity, type 2 diabetes, hypertension, and high cholesterol. A 2025 systematic review supports the association between fatty liver disease and metabolic risk factors, but does not diagnose fatty liver in an individual (PMID 40264655).

Differential diagnosis, in order of practical importance

Medication, supplement, or herbal-product liver injuryCommon and often missed. Review all prescribed medicines, OTC painkillers, gym/bodybuilding supplements, herbal products, antibiotics, antiepileptics, anti-TB drugs, and recent medication changes.
Chronic hepatitis B or CCan cause persistent mild ALT/AST elevations, sometimes without symptoms. Must be screened for rather than assumed absent.
Autoimmune hepatitisLess likely, but the raised globulin of 3.9 g/dL makes this worth considering if enzymes persist or rise. Check IgG, ANA, anti-smooth-muscle antibody, and liver-specialist review if indicated.
HemochromatosisIron overload can cause abnormal enzymes and metabolic-looking disease. Exclude with ferritin and transferrin saturation.
Wilson diseaseUncommon, but age 36 falls within the range where unexplained persistent enzyme elevation can warrant ceruloplasmin testing.
Muscle injury, strenuous exercise, or myopathyAST can come from muscle, but ALT and GGT being raised makes a liver source more likely. CK is useful if heavy exercise, muscle pain, or creatine supplements are involved.
Thyroid or celiac diseaseLess likely. Thyroid values are within this laboratory's reference interval, although TSH is toward the higher end. Consider celiac testing only if abnormal liver tests remain unexplained or symptoms/risk factors exist.
For mild, persistent elevations of aminotransferases, standard evaluation includes repeating tests, reviewing alcohol/medications/supplements, excluding muscle disease, and testing for hepatitis B/C, iron overload, autoimmune hepatitis, Wilson disease, and fatty liver disease. Sleisenger and Fordtran's Gastrointestinal and Liver Disease, p. 1395.

What is reassuring

  • No jaundice pattern: bilirubin normal.
  • No clear obstruction/bile-duct pattern: alkaline phosphatase normal.
  • Liver synthetic function appears preserved: albumin normal.
  • Kidney function normal: creatinine 0.78 mg/dL, eGFR 119 mL/min/1.73 m².
  • Thyroid hormones are in range.
These do not exclude fatty liver, chronic viral hepatitis, or early fibrosis, but make acute liver failure or significant biliary obstruction unlikely from this report alone.

Recommended next steps

Arrange a clinician visit within the next few weeks and discuss:
  1. Repeat liver panel in about 2-4 weeks, preferably avoiding alcohol, non-essential supplements, and intense exercise for several days beforehand.
  2. Ultrasound of abdomen/liver to assess steatosis and structural liver changes.
  3. Metabolic assessment: fasting glucose and HbA1c, blood pressure, waist circumference/BMI.
  4. Liver-cause screen:
    • HBsAg, anti-HBc, anti-HCV, with confirmatory tests if positive
    • Ferritin and transferrin saturation
    • IgG, ANA, anti-smooth-muscle antibody if persistent elevation or clinical suspicion
    • Consider ceruloplasmin if routine work-up is unrevealing.
  5. CBC with platelet count to calculate FIB-4, a preliminary fibrosis-risk score. It cannot be calculated from this report because platelet count is absent.
  6. Review cardiovascular risk and whether lipid-lowering treatment is appropriate. LDL 152 mg/dL merits formal risk assessment, especially with diabetes, hypertension, smoking, family history of premature heart disease, or obesity.
If the lipid sample was not collected after the requested 10-12 hour fast, triglycerides can be less reliable and should be repeated fasting.

Seek urgent care now if

Yellow eyes/skin, dark urine, persistent vomiting, severe right-upper-abdominal pain, confusion/drowsiness, abdominal swelling, bleeding/bruising, or rapidly worsening fatigue develops.
This report most strongly suggests metabolic fatty liver disease with atherogenic dyslipidemia, but hepatitis, alcohol exposure, medication/supplement injury, and autoimmune or iron-related disease should be actively ruled out before treating it as confirmed.

Liver evaluation checklist

Bring this list to your doctor. The priority is to confirm the abnormality, look for fatty liver, and exclude common treatable causes.

1. Repeat and confirm liver tests

  • Liver function panel:
    • ALT / SGPT
    • AST / SGOT
    • GGT
    • Alkaline phosphatase (ALP)
    • Total and direct bilirubin
    • Total protein and albumin
  • PT/INR, especially if bilirubin rises, there is bruising, jaundice, or more significant liver-test abnormality
  • Repeat after avoiding alcohol, unnecessary supplements, and strenuous exercise for several days, as advised by the clinician

2. Check for fatty liver and metabolic contributors

  • Ultrasound abdomen with liver assessment
  • Fasting blood glucose
  • HbA1c
  • Repeat fasting lipid profile
  • Blood pressure, BMI, and waist circumference
  • CBC with platelet count, needed to calculate FIB-4 fibrosis-risk score

3. Screen for viral hepatitis

  • Hepatitis B surface antigen, HBsAg
  • Hepatitis B core antibody, total anti-HBc
  • Hepatitis B surface antibody, anti-HBs - helps establish immunity
  • Hepatitis C antibody, anti-HCV
  • HCV RNA PCR only if anti-HCV is positive
  • HBV DNA only if hepatitis B markers suggest active or previous infection, as directed by the clinician

4. Screen for iron overload

  • Serum ferritin
  • Serum iron
  • Total iron-binding capacity, TIBC, or transferrin
  • Transferrin saturation

5. Autoimmune liver-disease assessment

The elevated globulin in this report makes this reasonable if enzymes remain elevated.
  • Serum immunoglobulin G, IgG
  • Antinuclear antibody, ANA
  • Anti-smooth muscle antibody, ASMA/SMA
  • Anti-liver-kidney microsomal type 1 antibody, anti-LKM-1, if advised by a specialist

6. Tests if the initial work-up is unrevealing or clinical suspicion exists

  • Ceruloplasmin, to screen for Wilson disease, particularly relevant in unexplained abnormalities at age 36
  • Alpha-1 antitrypsin level and phenotype/genotype
  • Creatine kinase, CK, if intense exercise, muscle pain/weakness, creatine use, or a muscle injury is possible
  • Celiac screen: tissue transglutaminase IgA and total IgA
  • Repeat TSH with free T4 if symptoms of thyroid dysfunction arise or values change

7. Fibrosis assessment, if fatty liver or persistent enzyme elevation is confirmed

  • Calculate FIB-4 using age, AST, ALT, and platelet count
  • Liver elastography/FibroScan if FIB-4 is indeterminate/high, ultrasound suggests chronic liver disease, or abnormal enzymes persist
  • Hepatology or gastroenterology consultation if results remain abnormal for more than 3-6 months, fibrosis risk is elevated, or a specific cause is identified

Clinical information to provide at the appointment

  • Exact alcohol intake per week, including binge episodes
  • All medicines, including recent antibiotics and pain medicines
  • All gym, protein, weight-loss, herbal, Ayurvedic, or bodybuilding supplements
  • Recent intense exercise or muscle injury
  • Diabetes, obesity, high blood pressure, sleep apnea, or family history of liver disease
  • Past blood transfusion, injections, tattoos/piercings, high-risk sexual exposure, or family hepatitis history
The core screening set is supported by the standard evaluation of persistent mild aminotransferase elevation: viral hepatitis B/C, fatty liver, iron overload, autoimmune hepatitis, and selected testing for Wilson disease and alpha-1 antitrypsin deficiency.
