The distinction is mainly dose and intended effect, not always the medication itself. Many drugs lie on a continuum:
  • Lorazepam at a lower dose may be used for calming anxiety or sedation; at a larger bedtime dose it may promote sleep.
  • Temazepam is a benzodiazepine mainly used as a hypnotic for insomnia.
  • Zolpidem is mainly a hypnotic, intended to induce sleep.
  • Midazolam is mainly used as a sedative for procedures.
So, sedative = calming, whereas hypnotic = sleep-inducing. Many medicines are called sedative-hypnotics because they can do both. Kaplan & Sadock’s Comprehensive Textbook of Psychiatry describes these effects as a continuum, and Goodman & Gilman’s The Pharmacological Basis of Therapeutics includes benzodiazepines, Z-drugs, and barbiturates in this group.
