Typical pediatric dosing is
6 to 7.5 mg/kg/day IV or IM, usually divided every 8 hours, equivalent to about
2 to 2.5 mg/kg per dose every 8 hours.

5 to 7 mg/kg/day.

Do not give netilmicin without a pediatric clinician’s prescription and monitoring. It can affect the kidneys and hearing, and the dose or interval must be adjusted in kidney impairment.
