Yes. Olanzapine can cause galactorrhea (milky nipple discharge) by increasing prolactin through dopamine D2 receptor blockade. It is generally less likely to raise prolactin than risperidone or paliperidone, but it can still happen, including after longer-term use. The FDA prescribing information specifically lists elevated prolactin and reports galactorrhea among possible associated effects.
What to do:
  • Do not stop olanzapine abruptly without speaking with the prescriber.
  • Contact the prescriber soon. They may check serum prolactin, a pregnancy test if applicable, and thyroid function, and review other medicines or causes.
  • Dose adjustment or a switch to a more prolactin-sparing option may be considered, but only by the treating clinician.
  • Seek urgent assessment if discharge is bloody, occurs from one breast only, there is a breast lump, severe headache, or vision changes.
A recent systematic review also supports that antipsychotics can produce dose-related prolactin changes, including with olanzapine (PMID 40830715).
