skull AP X-ray reporting systematic approach findings

Here is the formal radiology report for this skull X-ray:

X-Ray Skull - AP View (PA/Caldwell-like Projection)

Patient: Satyavati | Age/Sex: 36 Years / Female Study: Skull AP/Lateral | Date: 11-Jun-2026 Institution: Sri Raksha Hospitals, Khammam

RADIOLOGICAL REPORT

Projection: AP (anteroposterior) view of the skull
Technical Adequacy: Adequate positioning; the nasal septum is midline, indicating no rotation. The petrous ridges project symmetrically. Exposure is adequate.

SYSTEMATIC FINDINGS

1. Calvarium (Skull Vault)
  • The outer and inner tables are intact with no visible step-off, depression, or linear lucency to suggest a fracture.
  • Calvarial thickness is normal.
  • No lytic or sclerotic bony lesions are identified.
  • No "hair-on-end" or "beaten copper" trabecular pattern.
2. Sutures
  • Coronal and sagittal sutures are visible and within normal limits for age.
  • No diastasis of sutures.
  • No premature sutural fusion (craniosynostosis).
3. Sella Turcica / Intracranial Contents
  • The pituitary fossa (sella turcica) is not directly well-assessed on this projection; a lateral view would be required.
  • No abnormal intracranial calcifications are identified (no pineal, choroid plexus, or pathological calcifications visible on this view).
4. Orbits
  • Both orbital rims appear intact and symmetric.
  • No orbital wall disruption or "teardrop" sign to suggest orbital floor fracture.
5. Paranasal Sinuses
  • The frontal sinuses are visible bilaterally and appear normally pneumatized with no opacification or air-fluid levels.
  • The maxillary sinuses appear clear bilaterally.
  • The ethmoid air cells appear within normal limits.
  • No sinus wall disruption.
6. Nasal Cavity and Septum
  • Nasal septum appears midline.
  • No gross nasal bone fracture identified on this view.
7. Mandible
  • The mandibular body and condyles are partially visualized.
  • No obvious fracture or dislocation noted on this limited view.
  • A small metallic density is noted at the midline of the mandible/anterior teeth region - likely a dental restoration (metallic filling or dental wire).
8. Soft Tissues
  • No soft tissue swelling or subcutaneous emphysema visible.
  • No foreign bodies identified (apart from the probable dental restoration noted).

IMPRESSION

No acute bony abnormality identified on this AP skull radiograph.
  • Intact calvarium with no fracture, lytic/sclerotic lesion, or sutural diastasis.
  • Paranasal sinuses are clear; no opacification or air-fluid levels.
  • Orbits are symmetric with no evidence of fracture.
  • Small metallic density at the anterior mandible - consistent with a dental restoration (incidental finding).

RECOMMENDATION

  • A lateral skull view (already requested per "AP/LAT" label) should be correlated to assess the sella turcica, frontal/parietal/occipital bones in profile, and posterior fossa.
  • If clinical concern for facial fractures exists, a dedicated CT face/head is the investigation of choice for superior sensitivity.
  • Correlate with clinical history and neurological examination.

Note: This report is based on image analysis only. Final interpretation must be made by a qualified radiologist in the context of the patient's full clinical history.
