2026 SSC adult guideline key changes sepsis screening qSOFA antibiotics 3 hours possible sepsis capillary refill hydrocortisone fludrocortisone beta-lactam extended infusion

The 2026 Surviving Sepsis Campaign (SSC) adult guideline was published on March 23, 2026, in Critical Care Medicine and Intensive Care Medicine. It's the first major update since 2021. I read the guideline's recommendation tables, the SCCM quick guide, and secondary summaries, but not the full text. The library textbooks I searched only cite the 2016 and 2021 versions, so everything below comes from the 2026 sources.

Overview

  • It has 129 statements, 46 of them new. It was written by an international panel of more than 130 experts and is endorsed by 24 societies (ESICM).
  • The main themes are individualized care instead of fixed bundles, antimicrobial stewardship, fluid removal after resuscitation, and care after hospital discharge (The Hospitalist).
  • A pediatric guideline was released at the same time.

Antimicrobials

  • Septic shock: give antimicrobials immediately, ideally within 1 hour of recognition (strong, very low certainty).
  • Probable or definite sepsis without shock: give antimicrobials within 1 hour (strong).
  • Possible sepsis without shock: do a time-limited rapid work-up. If concern for infection persists, give antimicrobials within 3 hours of first suspicion (conditional). Clinicians should also rapidly assess whether the illness is infectious or non-infectious (good practice statement).
  • Empiric antifungals: suggest against routine use (conditional, low). Consider them case by case with immunosuppression, prolonged antibiotics or hospitalization, or an intra-abdominal source. Also suggest against using Candida biomarkers to start empiric antifungals (new).
  • Anaerobic coverage: new conditional recommendations were added on when to use it. I did not retrieve the details.

Fluids and perfusion monitoring

  • Initial fluids: at least 30 mL/kg IV crystalloid within 3 hours for sepsis-induced hypoperfusion or septic shock (conditional, low).
    • Reassess often to avoid under- or over-resuscitation.
    • Use actual body weight, or adjusted or ideal weight if BMI is above 30.
    • The panel notes this differs from the ESICM 2025 fluid guideline, which suggests "up to" 30 mL/kg (Springer).
  • Fluid type: use balanced solutions. Consider albumin as a supplement.
  • Monitoring: measure lactate and capillary refill time within 1 hour.
    • Serial lactate guides resuscitation (conditional). Fluids should be individualized after the initial bolus, not continued until lactate normalizes.
    • Capillary refill time is an adjunct to other perfusion measures (conditional).
    • Use dynamic measures of fluid responsiveness.
  • Active fluid removal is suggested after resuscitation (conditional), and a restrictive transfusion strategy is recommended (strong).

Blood pressure and vasopressors

  • MAP target: 65 mmHg (strong), with a remark that a range of about 5 mmHg is acceptable.
  • New for patients 65 or older with septic shock: suggest an initial MAP of 60 to 65 mmHg over higher ranges (conditional, low).
  • First-line agent: norepinephrine over dopamine, epinephrine, or selepressin (strong). It is also suggested over vasopressin or angiotensin II (conditional).
  • Escalation: add vasopressin if the norepinephrine dose is rising, then epinephrine as a third agent. In cardiac dysfunction, add dobutamine or use epinephrine.
  • Other agents: suggest against terlipressin. There is insufficient evidence on IV methylene blue.
  • Timing and access: consider starting norepinephrine alongside fluids when end-organ hypoperfusion is life-threatening. Start it peripherally rather than waiting for central access.
  • Hydrocortisone, with or without fludrocortisone, is used if the norepinephrine requirement persists (SCCM quick guide). I did not verify the dose or threshold wording.

Supportive care and after discharge

  • ICU admission within 6 hours is suggested if needed (conditional).
  • Start insulin at a glucose of 180 mg/dL or higher (strong).
  • Early enteral nutrition within 72 hours is suggested (conditional, very low certainty).
  • Sodium bicarbonate is suggested for septic shock with pH 7.2 or below and AKI (AKIN stage 2 or 3).
  • Pharmacologic VTE prophylaxis is recommended (strong), with low molecular weight heparin preferred over unfractionated heparin (strong). Pharmacologic prophylaxis alone is suggested over adding mechanical prophylaxis.
  • Discuss goals of care with patients and families, ideally within 72 hours.
  • The guideline adds recommendations on rehabilitation and care after discharge.

Controversy

Critics argue the guidelines have drifted toward mandated bundles and metric-based care. See the dissenting opinion in Am J Emerg Med (PMID 42767105). Other commentaries include a review of controversies and future directions (PMID 42747338) and a hospitalist summary (PMID 42904495).

Not verified

I did not check these topics against the primary text: sepsis screening tools, ventilation and ARDS, steroid dosing, and the anaerobic coverage criteria. Check the full guideline (DOI 10.1097/CCM.0000000000007075) before using any of them clinically.
I can go deeper on any section, such as antimicrobial timing, hemodynamics, or the critiques.

On the basis of this guidelines prepare a presentation

I've built a 16-slide deck from the 2026 Surviving Sepsis Campaign (SSC) adult guideline. I couldn't render it here, so I haven't seen the slides. A script check for text overflow found none, but flip through it for layout problems before presenting.

  15. Take-home messages
  16. References
Speaker notes with strength and certainty details are on most slides.
Gaps
  • I read the recommendation tables and summaries, not the full guideline text. Three items are not specified on the slides: steroid dose and threshold, the criteria for anaerobic coverage, and sepsis screening and ventilation recommendations. Slide 10 says to see the full text for the steroid details, and slide 5 does the same for anaerobic coverage.
  • The slide 14 timeline is my own synthesis of the recommendations and the SCCM quick guide, not a pathway the guideline gives. Check it against your local protocol.
  • The last line of slide 8 ("avoid chasing higher pressures...") is my interpretation of the lower MAP range for older adults, not guideline wording. Delete it if you want the slide to stick strictly to the recommendations.
I can add a slide on a topic I left out, change the design, or shorten the deck.

I still couldn't render the deck. A script check for text overflow found none, but check the new slide's layout, especially the table, when you open it.
Slide 11: Sepsis screening and respiratory support
Screening (left card, from the guideline's recommendations 1 and 4)
  • Hospitals should run a sepsis performance improvement program covering screening, standard operating procedures and quality improvement (strong).
  • Use NEWS, NEWS2, MEWS or SIRS over qSOFA as a single screening tool (strong, moderate certainty). qSOFA has poor sensitivity, but a positive result should still prompt a sepsis check.
  • For ambulance or air transport, use a standard screening tool such as NEWS2. The panel did not endorse one specific tool. One reason was a large prehospital cohort in which NEWS2 performed best but still had a positive predictive value of only 6.5%.
  • Consider a code sepsis or sepsis huddle protocol.
  • Sepsis is a clinical diagnosis, so no single biomarker rules it in or out.
Respiratory support (right table, recommendations 66 to 78)
TopicRecommendationStrength
Oxygen targetIndividualize SpO2, about 90-93% or 96% and above (new)Conditional
Initial supportHigh-flow nasal cannula (HFNC) over conventional oxygen, and over non-invasive positive pressure ventilation as the first step (new)Conditional
No ARDSTidal volume 6-8 mL/kg ideal body weight, and screen for ARDS regularly (new)Conditional
ARDS, volumeTidal volume 6 mL/kg, plateau pressure 30 cm H2O or lessStrong
ARDS, PEEPHigher PEEP; recommend against incremental PEEP titrationConditional / Strong
ARDS, proningProne more than 12 hours daily in moderate-severe ARDSConditional
ARDS, neuromuscular blockadeIntermittent boluses over continuous infusionConditional
Severe ARDSVeno-venous ECMO if conventional ventilation fails, in experienced centersConditional
The speaker notes carry the recommendation numbers and certainty levels.
Not covered on the slide
  • The guideline also has a conditional recommendation on awake proning. I only saw a remark that sedation should not be used to promote tolerance, so I left it out of the table.
  • The guideline also adds selective digestive tract decontamination for ventilated adults in units with low antimicrobial resistance. I saw this only in a secondary commentary, so I left it off the slide.
The "Not verified" caveat I gave earlier for screening and ventilation no longer applies. It still applies to steroid dosing and anaerobic coverage criteria.
