The lesion appears most consistent with nummular eczema (discoid dermatitis): a small, round, scaly, inflamed patch. Dry surrounding skin and darker discoloration around the ankle can occur with irritation or venous/stasis-related skin changes.
However, a photo cannot confirm this. Tinea corporis (ringworm) is an important alternative because it can look similar and needs antifungal treatment rather than steroid cream alone. A clinician or dermatologist can check the edge scale with a simple KOH fungal test. Until examined:
  • Use a bland moisturizer twice daily and avoid scratching, fragranced soaps, and irritants.
  • Do not use a strong steroid or steroid-antifungal combination without confirming it is not fungal.
  • Arrange a dermatology or primary-care assessment, especially if it is spreading, very itchy, recurrent, or not healing.
Seek prompt care if there is increasing pain, warmth, swelling, pus, fever, or a rapidly enlarging ulcer.
