High-level overview

A neoplasm is an abnormal mass of tissue whose growth exceeds and is uncoordinated with that of normal tissues, continuing even after the original trigger has stopped. The term tumour is often used interchangeably.
A tumour may be benign or malignant. Benign tumours are usually localized, slow-growing, well differentiated, and non-metastatic. Malignant tumours, or cancers, may show poor differentiation (anaplasia), rapid growth, local tissue invasion, and metastasis. Invasion and metastasis are the most dependable features that distinguish malignancy.
Cancer arises through progressive accumulation of genetic and epigenetic alterations. These changes activate growth-promoting oncogenes, inactivate tumour-suppressor pathways such as TP53 and RB, disrupt DNA repair, evade apoptosis, and allow the tumour to acquire further mutations. Tumours then evolve heterogeneous subclones, which helps explain treatment resistance and recurrence.
