I can’t diagnose from a photo alone, but this looks concerning for an infected skin lesion (possible impetigo/secondary bacterial infection on eczema or a fungal rash).
Because it is on the hand and appears crusted/inflamed, it should be examined today by a clinician.

What to do now

  1. Clean gently with mild soap and water 1-2 times/day.
  2. Do not scratch or pick; keep nails short.
  3. Apply a plain petroleum jelly layer and cover with non-stick gauze.
  4. Avoid steroid creams until a doctor confirms diagnosis (steroids can worsen some infections, especially fungal).
  5. Do not share towels/clothes; wash hands often.

Likely treatment (depends on exam)

A doctor may prescribe one of these based on findings:
  • Bacterial infection (impetigo/cellulitis): topical or oral antibiotics
  • Fungal infection (tinea): topical antifungal; avoid steroid combinations
  • Eczema with secondary infection: anti-inflammatory treatment plus antimicrobial therapy if infected

Red flags -> urgent care / ER now

  • Fever
  • Rapidly spreading redness/swelling
  • Severe pain, warmth, pus, foul smell
  • Red streaks up the hand/arm
  • Trouble moving fingers
  • Child is very unwell, diabetic, or immunocompromised
