Paramedic Fresh Job Plan - Step by Step

Phase 1 - Before Day One (Week -2 to 0)

• Submit: EMT/Paramedic license, CPR/ACLS/PALS cards, driver's license, background check, drug screen
• Confirm your National Registry certification (NREMT-P) is active
• Get immunizations up to date (Hep B, Tdap, flu, MMR, varicella) - required by most agencies

• Read the agency's protocols, formulary, and standard operating procedures (SOPs) cover to cover
• Know the medical director's name - they are legally responsible for your scope of practice
• Understand whether you work under an ALS or BLS system, and what your offline/online medical control procedures are

• Buy a quality stethoscope, trauma shears, penlight, gloves, and a pocket reference (like the Tarascon Paramedic Field Guide)
• Get comfortable boots - your feet will thank you

Phase 2 - First Week (Orientation)

• Most agencies have a 2-8 week FTO phase. Treat this as your most important stretch
• Ask your FTO to explain everything, even things you learned in school - every agency does things slightly differently
• Do NOT try to impress anyone. Focus on being reliable, safe, and a good partner

• Know where every piece of equipment is located on the unit - you must be able to find it at 3 AM with a critical patient
• Check the truck at the start of every shift (narcotics log, defibrillator, oxygen, IV supplies, airway equipment)
• Learn how to document on the agency's ePCR (electronic patient care report) system - this is huge

• Know your district: hospitals, trauma centers, stroke centers, cath labs, pediatric centers
• Know bypass protocols - not every hospital accepts every patient
• Learn major road landmarks, dead zones for radio, and closest ALS backup locations

Phase 3 - First 30-90 Days (Building Confidence)

• Do not try to be aggressive clinically before you have the fundamentals solid
• Focus on: patient assessment, IV access, airway management, 12-lead acquisition, medication dosing
• Develop your own systematic head-to-toe assessment routine and use it on every patient, every time

• Spend 15-20 minutes per shift reviewing one protocol (STEMI, sepsis, stroke, respiratory distress, cardiac arrest)
• Know your medication list cold: indications, contraindications, doses, routes, and side effects for every drug on your truck

• A good ePCR protects you legally and clinically
• Document what you found, what you did, and why - the "why" is what separates good paramedics from average ones
• Never alter a PCR after the fact

• Know your state/regional scope of practice and your medical director's specific protocols
• Understand informed consent, refusals, and the process for against-medical-advice (AMA) cases
• Know mandatory reporting laws (abuse, certain injuries)

Phase 4 - 3-6 Months (Clinical Growth)

• Ask your supervisor and medical director for case reviews
• Many agencies have a QI/QA (quality improvement) process - participate and learn from reviewed calls
• When you make an error, report it, learn from it, and move on

• Practice IV/IO access and intubation on simulators or in the OR if your agency offers clinical rotations
• Get comfortable with 12-lead ECG interpretation - this is the skill that makes or breaks ALS providers
• Learn to operate the monitor/defibrillator fluently (12-lead, pacing, cardioversion, pads vs paddles)

• Your EMT partner is your most important asset - treat them with respect
• Learn how to communicate effectively with ER nurses and physicians at handoff

• NREMT requires 60 CE hours every 2 years for recertification
