Free wall rupture that is CONTAINED by adherent pericardium - Wall = pericardium/scar (no myocardium) - HIGH risk of complete rupture - Requires surgical repair Ventricular Free Wall Rupture (5-14 Days) - Complete rupture of ventricular wall - Results in hemopericardium → cardiac tamponade - Often fatal - Protected by: previous MI (scar), LV hypertrophy True Ventricular Aneurysm (Weeks to Months) - Outward bulge of fibrotic scar - Dyskinesia (bulges OUT during systole, opposite of normal) - Wall = fibrotic myocardium (has all layers) - Lower rupture risk than pseudoaneurysm - Complications: mural thrombus → embolism, arrhythmias, heart failure
