For RBC transfusion in very preterm neonates, <30 weeks' gestation, use a restrictive hemoglobin-based threshold:

*Respiratory support: invasive ventilation, CPAP/NIPPV, or nasal cannula flow ≥1 L/min.
These thresholds are for stable anemia of prematurity. Transfuse outside them only after clinical assessment, such as significant acute blood loss, shock/hemodynamic instability, major hypoxemia, or symptomatic anemia. Use your NICU protocol for dose, usually packed RBC 10-15 mL/kg.
This restrictive strategy is recommended by the 2024 guideline based on six RCTs (3,483 infants), with no important short- or long-term outcome advantage from higher thresholds: JAMA guideline abstract.
