Adult outpatient hypertension treatment protocol

Do not use this for hypertensive emergency, pregnancy, or children. If BP is ≥180/120 mmHg with chest pain, dyspnea, neurologic deficit, confusion, acute visual change, or acute kidney symptoms, send for emergency assessment immediately.

1. Confirm and assess

  • Use correct cuff and repeat measurements on at least 2 occasions. Prefer validated home BP monitoring or 24-hour ambulatory monitoring to confirm persistent hypertension and identify white-coat hypertension.
  • Assess cardiovascular risk, target-organ damage, and secondary causes.
  • Baseline tests: creatinine/eGFR, electrolytes, urine albumin/protein, fasting glucose or HbA1c, lipids, ECG. Review BP-raising substances: NSAIDs, decongestants/stimulants, excess alcohol, cocaine/amphetamines, corticosteroids, oral contraceptives, and some herbal products.
  • Check adherence and home readings at every review. Home monitoring is recommended for initial assessment and follow-up. Katzung's Basic and Clinical Pharmacology, 16th ed., p. 290.

2. Treat everyone with lifestyle measures

Start at diagnosis and continue alongside medication:
  • DASH-style diet: vegetables, fruits, legumes, whole grains, low-fat dairy; reduce processed foods.
  • Reduce sodium, aim for roughly <1.5 to 2 g sodium/day when feasible.
  • Weight reduction if overweight.
  • Aerobic plus resistance exercise, about 150 minutes/week of moderate activity if safe.
  • Limit alcohol, stop smoking, optimize sleep, and evaluate/treat obstructive sleep apnea when indicated.

3. Medication initiation

The 2025 ACC/AHA multisociety approach uses a general treatment goal of <130/80 mmHg and recommends medication for persistent 130-139/80-89 mmHg after 3-6 months of lifestyle therapy even in lower-risk adults. See the ACC guideline summary.

4. Choose first-line medicines

Use one, or usually two, of the following:
  1. Thiazide or thiazide-like diuretic
    Examples: chlorthalidone, indapamide, hydrochlorothiazide.
    Monitor: sodium, potassium, uric acid, renal function.
  2. ACE inhibitor
    Examples: lisinopril, enalapril.
    Avoid in pregnancy; monitor potassium and creatinine. Do not combine with an ARB.
  3. ARB
    Examples: losartan, valsartan.
    Useful when ACE inhibitor cough or angioedema intolerance occurs. Avoid in pregnancy; monitor potassium and creatinine.
  4. Long-acting dihydropyridine calcium-channel blocker
    Examples: amlodipine, felodipine.
    Monitor for ankle edema, flushing, headache.
Current ACC guidance continues to prioritize thiazide-type diuretics, long-acting dihydropyridine calcium-channel blockers, and ACE inhibitors or ARBs as first-line therapy, favoring once-daily and single-pill regimens where possible. See the ACC medication recommendations.
Common two-drug combinations
  • ACE inhibitor or ARB + amlodipine
  • ACE inhibitor or ARB + thiazide/thiazide-like diuretic
  • Amlodipine + thiazide/thiazide-like diuretic
Avoid: ACE inhibitor + ARB together.

5. Tailor to comorbidities

  • CKD with albuminuria: ACE inhibitor or ARB, unless contraindicated.
  • Coronary disease, previous MI, angina, HFrEF, rate-control indication: beta-blocker may be indicated, but it is not routine first-line therapy for uncomplicated hypertension.
  • Heart failure with reduced ejection fraction: use guideline-directed heart-failure therapy, including appropriate beta-blocker, ACE inhibitor/ARB/ARNI, mineralocorticoid receptor antagonist, and SGLT2 inhibitor as clinically indicated.
  • Diabetes: ACE inhibitor or ARB is preferred if albuminuria is present.
  • Gout: minimize or avoid thiazide if it worsens hyperuricemia.
  • Pregnancy or planning pregnancy: avoid ACE inhibitors, ARBs, and direct renin inhibitors. Use an obstetric-specific plan such as labetalol, nifedipine, or methyldopa where appropriate.
  • Black adults without CKD/HF: a thiazide-type diuretic and/or dihydropyridine CCB is often an effective initial choice.

6. Follow-up and titration

  • Review about monthly after starting or changing therapy until controlled.
  • Check home BP log, side effects, adherence, and lifestyle barriers.
  • Recheck creatinine and potassium about 1-4 weeks after starting or increasing an ACE inhibitor, ARB, or diuretic.
  • Titrate dose or add another first-line drug every 2-4 weeks until at goal.
  • Once controlled, review every 3-6 months.

7. Resistant hypertension pathway

Consider resistant hypertension when BP remains above target despite adherence to three drugs at appropriate doses, typically including:
  • ACE inhibitor or ARB
  • Long-acting CCB
  • Thiazide-like diuretic
Then:
  1. Confirm technique, home/ambulatory BP, and adherence.
  2. Remove BP-raising agents and excess sodium/alcohol.
  3. Screen for secondary causes, especially primary aldosteronism, CKD, renal artery disease, and obstructive sleep apnea.
  4. Add a mineralocorticoid receptor antagonist such as spironolactone if renal function and potassium permit.
  5. Refer to a hypertension, nephrology, or cardiology specialist if still uncontrolled.

Practical target

For most nonpregnant adults: BP <130/80 mmHg if tolerated, while avoiding symptomatic hypotension, falls, acute kidney injury, or medication harms. The 2024 ESC guideline page is also a current reference for diagnosis and treatment pathways.
Recent evidence supports intensive BP targets in appropriate patients, but target selection must be individualized for frailty, orthostatic symptoms, kidney function, and competing risks. Recent systematic reviews include PMIDs 40172433 and 40132860.

Clonidine group: centrally acting sympatholytic antihypertensive, specifically a central alpha-2 adrenergic receptor agonist.
It lowers sympathetic outflow from the brainstem, reducing heart rate, cardiac output, and peripheral vascular resistance.
Other drugs in this group:
  • Methyldopa: central alpha agonist. Commonly used for hypertension in pregnancy.
  • Guanfacine: central alpha-2 agonist, similar to clonidine.
  • Guanabenz: central alpha-2 agonist, now rarely used.
  • Moxonidine and rilmenidine: centrally acting imidazoline I1 receptor agonists with some alpha-2 activity. Used in some countries.
Important adverse effects of clonidine and related drugs
  • Sedation, fatigue
  • Dry mouth
  • Bradycardia and hypotension
  • Constipation
  • Sexual dysfunction
  • Rebound hypertension if stopped abruptly. Taper gradually, particularly if also taking a beta-blocker.
Clonidine is generally an add-on option for resistant hypertension, not a usual first-line antihypertensive. The NCBI drug summary similarly classifies it as a centrally active alpha-adrenergic agonist.
