This is the 2025 ATA Guidelines for Adult Differentiated Thyroid Cancer (DTC). It runs 145 pages and has 80 numbered recommendations (R1 to R80). A single memory sheet would be too thin or too long to be useful, so I'll split it into topics and go deep on the one you pick.

Topic map

  1. DATA framework, pathology, NIFTP, epidemiology (R1)
  2. Genetics: germline testing and familial non-medullary thyroid cancer (R2 to R5)
  3. Pre-op work-up: imaging, Tg, molecular testing, active surveillance and ablation (R6 to R14)
  4. Surgery: lobectomy versus total thyroidectomy, completion surgery, neck dissection, thyroglossal duct carcinoma (R15 to R20)
  5. Voice and parathyroid protection, histopathology (R21 to R27)
  6. Post-op risk stratification and response to therapy: recurrence risk, Tg timing, imaging (R28 to R31)
  7. Radioiodine (RAI): who gets it, preparation, low-iodine diet, scans, safety, side effects, pregnancy, and the role of external radiation (R32 to R44)
  8. Long-term management: TSH targets and duration, Tg follow-up, de-escalating monitoring, ongoing risk reassessment (R45 to R51)
  9. Recurrent and metastatic disease: nodal, lung and bone metastases, RAI dosing, RAI-refractory (RAIR) definition (R52 to R60)
  10. Systemic therapy for RAIR DTC: lenvatinib and sorafenib, NTRK, RET, ALK and BRAF-directed drugs, immunotherapy, redifferentiation, brain and bone metastases, clinical trials (R61 to R80)
  11. Remaining sections: patient-reported outcomes and research gaps

High-level overview

  • DTC means papillary, follicular and oncocytic carcinomas, which make up more than 90% of thyroid cancers. The prognosis is generally excellent. Incidence tripled from 1975 to 2015, driven largely by ultrasound-detected small tumors. It has fallen since 2014 because guidelines now discourage FNA of nodules under 1 cm without abnormal nodes or invasion.
  • The new idea is "DATA": Diagnosis, risk/benefit Assessment, Treatment decisions, response Assessment. You use it at diagnosis and again at any recurrence. The aim is to match treatment intensity to risk. Do less for low-risk disease and more for high-risk disease or an inadequate response.
  • The practical flow of care is: stage and risk-stratify before surgery, operate, re-stratify after surgery, decide on RAI, suppress TSH only as much as the risk requires, monitor with Tg and ultrasound, and move to targeted systemic therapy only for progressive RAIR disease.
Which topic do you want first? I'll turn it into a mnemonic-driven memory sheet with stories, numbers to remember, comparison tables and likely exam traps.

I can't build the full set of memory notes yet. The PDF is no longer in my workspace. ATA DTC 2025.pdf was there earlier and I read it, but the attachments folder is now empty and the file won't open. If I carried on, I'd be writing from memory instead of from your document, and that could introduce errors into something you're studying from.
Please re-upload ATA DTC 2025.pdf. Once it's back, I'll build the whole thing in one pass, covering all 80 recommendations (R1 to R80) and the key tables. I'll deliver it as a downloadable PDF, organised like this:
  • A one-page "DATA" map of the whole patient journey (Diagnosis, risk/benefit Assessment, Treatment decisions, response Assessment).
  • Topic by topic (pathology and NIFTP, genetics, pre-op work-up, surgery, voice and parathyroid, risk stratification, radioiodine, TSH suppression and follow-up, recurrence, and RAI-refractory targeted therapy), each with:
    • a short story or scenario that anchors the logic,
    • a mnemonic for the lists,
    • the numbers and cut-offs worth memorising,
    • "if X then Y" decision rules,
    • exam traps.
  • Comparison tables for risk categories, response-to-therapy categories, TSH targets by risk, and the drug matched to each driver alteration (NTRK, RET, ALK, BRAF).
  • A final recall sheet of rapid-fire questions with answers, so you can test yourself.

I've re-uploaded the PDF, please build the full memory notes

I haven't looked at the rendered pages, only the extracted text, so skim the layout when you open it.
How it works: one patient, Mira, 38, goes through the whole guideline in 11 chapters. Each chapter has:
  • a short story that carries the logic,
  • a table of the recommendations with their strength (Strong, Conditional or Good Practice Statement),
  • the numbers to memorise,
  • memory hooks,
  • exam traps.
Chapters:
  1. Pathology and NIFTP
  2. Genetics
  3. Pre-op work-up and active surveillance
  4. Extent of surgery
  5. Voice and parathyroid protection
  6. Risk and response after surgery
  7. Radioiodine (RAI)
  8. TSH, thyroglobulin and ultrasound follow-up
  9. Recurrence and metastases
  10. RAI-refractory disease and targeted therapy
  11. Pregnancy and survivorship
Also included:
  • Two colour-coded tables: the 2025 ATA risk of recurrence (read from Figure 2 of the guideline) and the response-to-therapy criteria (Table 9).
  • A list of every Strong recommendation, which is the high-yield list for exams.
  • A "NO list" of things the guideline says not to do routinely.
  • A 50-question Recall Arena where you cover the answer column and test yourself.
A few hooks to start with:
  • Risk ladder "10-15-30": low is under 10% recurrence, low-intermediate 10 to 15%, intermediate-high 16 to 30%, high over 30%.
  • Thyroglobulin (Tg) ladder:
    • After total thyroidectomy with RAI: "0.2 - 1 - 10".
    • After total thyroidectomy without RAI: "2.5 - 5".
  • Surgery size "2-4-Total": lobectomy up to 2 cm, either operation from 2 to 4 cm, and total thyroidectomy beyond 4 cm, with gross extrathyroidal extension, nodal disease or distant metastases.
  • Dose ladder "30-100-150-200": RAI in mCi for low, intermediate, high and metastatic disease.
  • Targeted-therapy rule "NRA": NTRK, RET and ALK fusions get a matched drug in first line. BRAF V600E is the exception.
Things to know:
  • The mnemonics and stories are my own study aids. They are not text from the guideline.
  • Drug names for NTRK, RET, ALK and BRAF come from my general knowledge, not from your PDF, and they are labelled that way in the notes.
  • I did not search PubMed for newer evidence, so everything comes from your PDF.
  • Check clinical decisions against the full guideline.
I can add a one-page poster version, flashcards, or a deeper drill on any chapter if you want one.
