Key Notes

  • H + Pyridoxine (Vitamin B6): Always co-prescribe pyridoxine 25-50 mg/day with Isoniazid to prevent peripheral neuropathy (especially in malnourished, pregnant, diabetic, HIV+ patients)
  • E (Ethambutol): Monitor visual acuity and color vision; avoid in young children who cannot report visual changes
  • R (Rifampicin): Strong enzyme inducer - orange discoloration of body fluids, multiple drug interactions (especially with HIV ARVs)
  • Z (Pyrazinamide): Monitor uric acid and liver enzymes; causes hyperuricemia and hepatotoxicity
  • Standard regimen: 2HRZE (intensive phase, 2 months) + 4HR (continuation phase, 4 months) = 6 months total
  • Pyridoxine (Vitamin B6) dose by weight (India): 50 mg for 16-29 kg; 100 mg for >30 kg
