Prostatomegaly means an enlarged prostate, generally defined on ultrasound as a volume over 30 mL (cc).
There is no universally standardized grading system, but a commonly used ultrasound reporting convention is:
| Prostate volume | Usual description |
|---|---|
| Up to 30 mL | Normal / not enlarged |
| 31-50 mL | Mild prostatomegaly |
| 51-80 mL | Moderate prostatomegaly |
| Over 80 mL | Severe / marked prostatomegaly |
Some radiology departments instead use:
- Mild: 30-40 mL
- Moderate: 40-60 mL
- Severe: >60 mL
So, interpret the stated grade according to the reporting center and the actual prostate volume in mL. Ultrasound estimates volume as length × width × height × 0.52. An enlarged prostate does not by itself prove BPH, and symptoms, post-void residual urine, PSA when appropriate, and clinical examination guide evaluation. Radiopaedia’s prostatomegaly reference notes the usual imaging threshold of >30 mL.
