The parasite digests host red cell hemoglobin to extract amino acids, releasing large amounts of free heme as a byproduct. Free heme is toxic to the parasite, so it normally polymerizes heme into the inert crystal hemozoin in its food vacuole. Chloroquine, being a diprotic weak base, concentrates in the acidic food vacuole via ion trapping and specifically binds to heme, blocking its polymerization to hemozoin. The resulting accumulation of free heme causes oxidative damage to phospholipid membranes, leading to lysis of both the parasite and the red blood cell.

Clinical Uses
- Treatment of malaria - drug of choice for uncomplicated non-falciparum and chloroquine-sensitive P. falciparum malaria. Rapidly terminates fever (24-48 hours) and clears parasitemia (48-72 hours).
- Chemoprophylaxis - preferred agent in malarious regions without chloroquine-resistant P. falciparum.
