Classification
Pulp stones are classified in two ways:
1. According to Structure:
  • True pulp stone - It has a similar structure to dentin and contains dentinal tubules with odontoblastic processes. True pulp stones are rare and are located close to the apical foramen. They are theorized to be formed by the inclusion of Hertwig's Epithelial Root Sheath (HERS) within the pulp, where HERS induces the cells of the pulp to form dentin masses, resulting in a true pulp stone.
  • False pulp stone - It does not exhibit dentinal tubules and appears as a concentric layer of calcified mass. It may appear within bundles of collagen fibers or in pulp free of collagen. Some may arise around blood vessels, and calcifications of thrombi in blood vessels, called phleboliths, may serve as nidi for false pulp stones.
2. According to Their Relation to Dentin:
  • Free pulp stones are entirely surrounded by pulp tissue.
  • Attached pulp stones are partly fused with dentin.
  • Embedded pulp stones are entirely surrounded by dentin.
All three types are believed to be formed freely and later become surrounded by dentin. The incidence and size of pulp stones increase with age.
