Non-Healing Corneal Ulcer kya hota hai?

Corneal ulcer tab non-healing consider kiya jaata hai jab standard treatment ke baad bhi 2-3 weeks mein healing nahi hoti. Iska ek aur term hai Indolent ulcer ya Persistent epithelial defect (PED).

Causes - Enumerated (Yaad rakhne ke liye categories mein)

1. INFECTIVE CAUSES (Organisms jo difficult-to-treat hain)

2. NEUROTROPHIC KERATOPATHY (Sabse important cause!)

Kanski ke anusaar: "Neurotrophic keratopathy occurs when there is loss of trigeminal innervation to the cornea resulting in partial or complete anaesthesia... leading to persistent ulceration."
Causes of neurotrophic keratopathy:
  • Trigeminal ganglion surgery (neuralgia ke liye)
  • Herpes simplex / Herpes zoster keratitis (sectoral sensation loss)
  • Stroke / Brain tumor (intracranial)
  • Diabetes mellitus (peripheral neuropathy)
  • Long-term contact lens use
  • Prior ocular surgery
  • Chemical injury

3. EXPOSURE KERATITIS (Lagophthalmos)

Aankhein poori band nahi ho paati toh cornea dry ho jaata hai:
  • Bell's palsy (7th nerve palsy)
  • Cicatricial lid conditions (burns, trachoma)
  • Proptosis / Exophthalmos (thyroid eye disease)
  • Post-operative ectropion
  • Unconscious/comatose patients mein

4. DRY EYE DISEASE (Keratoconjunctivitis Sicca)

  • Sjogren's syndrome
  • Meibomian gland dysfunction
  • Post-LASIK dry eye
  • Vitamin A deficiency (Keratomalacia)
  • Radiotherapy effects

5. LIMBAL STEM CELL DEFICIENCY

  • Cornea ki repair cells khatam ho jaati hain
  • Causes: Chemical/thermal burns, Stevens-Johnson Syndrome, Ocular cicatricial pemphigoid, aniridia

6. TOPICAL MEDICATION TOXICITY

  • Topical steroids ka overuse → healing impair karta hai, secondary infection bhi promote karta hai
  • Topical anesthetics (e.g., proparacaine) - prolonged use blocks epithelial healing
  • Topical NSAIDs overuse
  • Preservatives (e.g., benzalkonium chloride) wali eye drops

  • Entropion / Trichiasis - eyelashes continuously cornea ko scratch karte hain
  • Foreign body retained in cornea ya subtarsal
  • Symblepharon - conjunctival adhesions
  • Pterygium overlying ulcer
  • Previous corneal surgery - altered innervation/blood supply

9. IMMUNOCOMPROMISED STATE

  • HIV/AIDS patients
  • Long-term systemic steroid use
  • Post-transplant immunosuppression
  • Malignancy patients (on chemotherapy)

10. ISCHEMIC CAUSES

  • Anterior segment ischemia
  • Peripheral ulcerative keratitis secondary to vasculitis

  1. Neurotrophic ulcer - Kanski mein clearly mention hai ki trigeminal loss → persistent ulceration. Yeh MBBS exam mein frequently puchha jaata hai.
  2. Acanthamoeba - Contact lens users mein non-healing ulcer ka common cause; ring-shaped infiltrate pathognomonic hai.
  3. Fungal ulcer - Feathery edges, satellite lesions, slow healing.
  4. Topical steroid misuse - India mein bahut common cause of non-healing ulcer.
  5. Peripheral ulcerative keratitis (PUK) - Always systemic vasculitis workup karo (RA, Wegener's).
