Coup and contrecoup injuries

Both are brain contusions caused by head trauma.

Mechanism: The brain moves within the skull after sudden acceleration or deceleration. It can strike the inner skull surface, producing bruising or hemorrhagic contusions. Irregular bony areas of the anterior and middle cranial fossae make the frontal and temporal poles especially susceptible.
Examples
  • Blow to the occiput: coup contusion at the occipital region, with possible contrecoup contusions in the frontal poles.
  • Fall backward onto the occiput: contrecoup frontal contusions may predominate.
  • A person may have only a contrecoup lesion, and skull fracture is not required. The frontal lobes are common sites for contrecoup injury. P C Dikshit Textbook of Forensic Medicine and Toxicology, lines 3602-3624.
Medicolegal point: The pattern may suggest the mechanism of injury, but it does not by itself prove whether trauma resulted from a fall, assault, or another event.
