Indications for Heparin (Inj.) in Pregnancy
Heparin (preferably Low Molecular Weight Heparin / LMWH) is the anticoagulant of choice during pregnancy because it does not cross the placenta. Warfarin/VKAs are contraindicated due to teratogenicity and fetal hemorrhage risk.

Why Heparin Over Warfarin in Pregnancy?
- Does not cross placenta → no fetal anticoagulation
- No teratogenicity
- Reversible with protamine sulfate
- LMWH preferred over UFH due to predictable dosing, lower risk of HIT and osteoporosis
LMWH is also compatible with breastfeeding.
