Indications for Heparin (Inj.) in Pregnancy

Heparin (preferably Low Molecular Weight Heparin / LMWH) is the anticoagulant of choice during pregnancy because it does not cross the placenta. Warfarin/VKAs are contraindicated due to teratogenicity and fetal hemorrhage risk.

Key Indications:

IndicationDetails
Deep Vein Thrombosis (DVT)Treatment & prevention of DVT during pregnancy
Pulmonary Embolism (PE)Therapeutic anticoagulation
Antiphospholipid Syndrome (APS)Prevention of recurrent pregnancy loss & thrombosis (with aspirin)
Atrial FibrillationThromboembolism prophylaxis
Mechanical Heart ValvesAnticoagulation when warfarin is switched during pregnancy
Recurrent MiscarriageThrombophilia-associated pregnancy loss (e.g., Factor V Leiden, prothrombin mutation)
Prior VTE (history)Prophylaxis in subsequent pregnancy
Portal/Hepatic Vein ThrombosisLMWH is anticoagulant of choice (VKAs contraindicated)
Pre-eclampsia prophylaxisIn high-risk cases with thrombophilia
Placental insufficiencyIn selected thrombophilia cases

Why Heparin Over Warfarin in Pregnancy?

  • Does not cross placenta → no fetal anticoagulation
  • No teratogenicity
  • Reversible with protamine sulfate
  • LMWH preferred over UFH due to predictable dosing, lower risk of HIT and osteoporosis
LMWH is also compatible with breastfeeding.
