ISGPS postoperative pancreatic fistula definition drain amylase day 3 2016 Whipple drain output POD 0

On POD 0 after a Whipple, there is no single drain-volume cutoff that defines a pancreatic leak. Drain volume alone is less important than rate, colour, and the patient’s condition.
Treat as urgent and inform the operating surgeon immediately if:
  • Fresh bright-red blood or rapidly increasing sanguineous output
  • Rough guide: >100 mL/hour of fresh blood for 2 consecutive hours, or any sudden brisk output
  • Total output approaching 500 mL of blood in a short period, especially in this 40 kg patient
  • Falling BP, tachycardia, reduced urine output, rising drain bloodiness, increasing abdominal distension, or a falling Hb
For a 40 kg adult, estimated blood volume is roughly 2.4 to 2.8 L. Thus, a 500 mL acute loss is about 18% to 20% of blood volume, which is clinically important.
Non-bloody output
  • Small to moderate serosanguinous drainage on POD 0 can be expected.
  • Bilious, green, feculent, cloudy/purulent, or suddenly high-volume output also warrants surgical review.
  • Pancreatic fistula cannot be diagnosed on POD 0. ISGPS assesses measurable drain fluid with amylase >3 times the upper normal serum amylase from POD 3 onward, with clinical relevance, as summarized in the ISGPS definition. Surgical texts similarly emphasize amylase-rich drain fluid for pancreatic disruption rather than output volume alone - Fischer's Mastery of Surgery, 8th ed., pancreatic complications section.
Practical action now: record hourly drain volume and appearance, check vitals and urine output, inspect Hb/coagulation as ordered, and escalate any fresh blood or hemodynamic change without waiting for a numeric threshold. Do not clamp or remove the drain unless the surgeon directs it.
