1. Prostate cancer frequently metastasizes to the vertebrae.

The prostate has veins (Batson's venous plexus) that connect directly to the veins around the spine. There are no valves in these veins, so cancer cells can easily flow backward into the vertebrae. That's why back pain is often the first sign of prostate cancer spread.

2. Periumbilical pain (e.g., in appendicitis)

The appendix and the gut are supplied by nerves that enter the spinal cord at the T10 level - the same level that supplies the area around the belly button (umbilicus). So early pain from the appendix is "felt" at the belly button before it moves to the right lower side later.

3. Gastric ulcer more commonly found along the lesser curvature of stomach

The lesser curvature has the least muscular protection and the food slides along it the most (it's like the "fast lane" of the stomach). Also, the blood supply is less robust here, making it more vulnerable to acid damage.

4. Fibula more commonly used for bone grafting

The fibula is the thin bone of the leg. You don't need it to walk - the tibia (big leg bone) does all the weight-bearing. So surgeons can take a piece (or even the whole fibula) without affecting leg function. It's also long, has its own blood supply, and can be shaped easily.

5. Saphenous vein commonly used for CABG (heart bypass)

The great saphenous vein runs from your foot to your groin. It's:
  • Long enough to use as a bypass graft
  • Easy to access (just under the skin)
  • Expendable - you can live without it because other veins take over drainage
  • Strong-walled and good diameter

6. Varicocele is more common on the LEFT side

A varicocele = swollen veins in the scrotum (like varicose veins but in the testicles).
  • Left testicular vein drains into the left renal vein at a 90° angle (like a T-junction) - this creates back pressure and slows blood flow
  • Right testicular vein drains directly into the inferior vena cava at an angle - flows more easily
  • Also, the left renal vein can get compressed between the aorta and superior mesenteric artery ("nutcracker effect")
Result: blood pools in the left testicular veins → varicocele

7. Embryological basis of omphalocele

During fetal development (around week 6), the gut grows so fast that it temporarily herniates out of the belly into the umbilical cord. Normally, by week 10, it returns back inside. If it fails to return, the baby is born with gut outside the belly covered only by a thin membrane - this is called omphalocele. It's a failure of the gut to return to the abdomen.

8. Pregnant women have predominantly thoracic (chest) type of respiration

Normally we breathe using our diaphragm (abdominal breathing). But in pregnancy:
  • The growing uterus pushes up against the diaphragm
  • The diaphragm can't move down freely
  • So the woman switches to breathing by expanding her chest (ribs) instead - called thoracic breathing

9. Outer quadrant of gluteal region is commonly used for intramuscular injections

The buttock has the sciatic nerve running through it - the largest nerve in the body. It passes through the lower and inner parts of the buttock. The upper outer quadrant is safely away from the sciatic nerve, so injecting there avoids nerve damage (which could cause foot drop).

10. Fluid accumulates in the Pouch of Douglas in ambulatory (standing/walking) patients

The Pouch of Douglas = the lowest point in the female pelvis (space between uterus and rectum). When you're upright (standing or walking), gravity pulls fluid downward. Since this pouch is the lowest point in the pelvic/abdominal cavity when standing, fluid naturally collects there. That's where doctors look for free fluid in the pelvis.

11. Foot drop occurs in fracture of neck of fibula

The common peroneal nerve winds tightly around the neck of the fibula (just below the knee). If the fibula breaks here, the nerve gets damaged. This nerve controls the muscles that lift your foot up. Without it, the foot just hangs down = foot drop (you drag your toes when walking).

12. Inhaled foreign body tends to pass into the RIGHT lung

The right main bronchus (airway to right lung) is:
  • More vertical (straighter) than the left
  • Wider in diameter
  • Shorter
So anything you inhale - a peanut, a coin, etc. - tends to follow the straighter, wider path into the right lung, just like water flows down the straightest path.

13. Soleus muscle is known as the "peripheral heart"

The soleus muscle in the calf constantly contracts during standing. The veins in the calf (especially the soleal sinuses) hold a lot of blood. When soleus contracts, it squeezes these veins, pumping blood back up toward the heart against gravity. It acts like a second pump - hence "peripheral heart." This is also why prolonged sitting (no calf muscle pumping) causes DVT.

14. Femoral hernia is more common in females

The femoral canal is the space through which the femoral vessels pass into the thigh. In females:
  • The pelvis is wider
  • The femoral canal is larger/wider
  • More space = easier for bowel or fat to slip through
  • Also, pregnancy increases abdominal pressure, pushing things downward

15. Pain of appendicitis felt at the umbilicus (early)

Same as Q2 - the appendix is supplied by T10 nerves, which also supply the skin around the belly button. So early appendicitis pain is felt at the umbilicus. Later (hours), it shifts to the right iliac fossa as the overlying peritoneum gets inflamed and more specific local nerves take over.

16. Varicoceles are more common on the LEFT side of the body

(Same as Q6 - repeated topic) Left testicular vein drains into the left renal vein at a right angle → high resistance → blood backs up → varicocele. Right side drains at an angle into IVC → less backpressure.

17. Pain of hip joint is felt at the knee

The obturator nerve supplies both the hip joint and sends a branch to the knee joint. So when there's hip disease (e.g., hip arthritis in a child), the brain gets confused and "thinks" the pain is coming from the knee. This is called referred pain. A child complaining of knee pain should always get a hip examination!

18. Caput medusae is seen at the umbilicus

Caput medusae = dilated veins radiating from the belly button, looking like snake hair (like Medusa's head).
Cause: In liver cirrhosis, blood can't flow through the liver. It backs up and finds alternative routes. One route is through the para-umbilical veins (around the belly button) to the skin veins. These veins enlarge visibly around the umbilicus.

19. Femoral hernia is more common in females than males

(Repeated from Q14) Wider female pelvis → larger femoral canal → easier for herniation. Plus pregnancy-related increased abdominal pressure.

20. Thorax is circular in children, kidney-shaped in adults

In infants/children: ribs are more horizontal, chest is round like a barrel when seen from above (circular cross-section).
In adults: ribs slope downward, chest becomes flatter front-to-back and wider side-to-side - giving a kidney-bean/oval shape on cross-section.
This also explains why babies breathe mostly with their abdomen (diaphragm) - their ribs can't expand much side-to-side.

21. Referred pain of gallbladder felt at the tip of the right shoulder

The gallbladder sits under the liver and when inflamed, it irritates the diaphragm above it. The diaphragm is supplied by the phrenic nerve (C3, C4, C5). The same C3-C5 nerves also supply the skin over the right shoulder. So the brain thinks the pain is coming from the shoulder = referred pain at the tip of right shoulder.

22. Varicocele is more common on the left side

23. Perineal body tear may lead to prolapse of pelvic organs

The perineal body is a fibromuscular knot of tissue between the vagina and anus. It's the anchor point for many pelvic floor muscles. During difficult childbirth, it can tear. If it tears:
  • The pelvic floor loses its support structure
  • Muscles and ligaments become weak
  • The uterus, bladder, or rectum can then bulge downward = prolapse

24. Anatomical basis for spread of breast carcinoma

Breast cancer spreads mainly through lymphatics:
  • 75% drains to axillary lymph nodes (armpit) → most common spread
  • Also to internal mammary nodes (beside the sternum)
  • Also to supraclavicular nodes

Blood spread goes to: bones (vertebrae, ribs), lungs, liver, brain.

The hip sacrifices mobility for stability. The shoulder does the opposite - very mobile but dislocates easily.

28. Twinning in pregnancy

Two types:
  • Identical (monozygotic): One egg + one sperm → splits into two. Always same sex. Share placenta (sometimes).
  • Fraternal (dizygotic): Two eggs + two sperms → two separate babies. Can be different sexes. Separate placentas.
Dizygotic twins are more common and run in families (on the mother's side - she releases two eggs). Monozygotic is random.

29. Great saphenous vein commonly involved in varicosity

(Related to Q5) The great saphenous vein runs just under the skin of the leg. It has valves that prevent blood from flowing backward. When these valves fail:
  • Blood pools in the vein
  • Vein walls stretch and become tortuous = varicose veins
It's the most superficial large vein in the leg, most exposed to gravity pressure, and valves fail with prolonged standing, pregnancy, or obesity.
