  • The diatom test is the most reliable test to confirm antemortem drowning - diatoms crossing into distant organs (BM, spleen, kidney) via the bloodstream proves the person was alive while drowning.
  • In hanging, the hyoid bone fractures more often (due to the upward pull of the ligature), while the thyroid cartilage fractures more in strangulation (direct compression).
  • The ligature mark in hanging is oblique and incomplete (does not go all the way around), whereas in strangulation it is horizontal and complete.
  • Saliva stains and seminal discharge are common in hanging (vagal stimulation), rare in strangulation.
