Case Summary
Mr. Inder Lal is a 77-year-old male admitted to the Cardiology ICU with acute coronary syndrome (ACS). He presented with chest pain and sweating 4 days prior to admission, followed by two episodes of giddiness. At NHI MMI Hospital, he was diagnosed with ACS with positive troponin I and was managed according to the ACS protocol.
Coronary angiography revealed triple-vessel coronary artery disease (TVD). He is a known case of diabetes mellitus and hypertension, both reportedly controlled on medication.
He has been shifted to the Cardiology ICU for further management and revascularization planning. At present, he is hemodynamically stable, conscious, and oriented, with no requirement for inotropic support.
Provisional diagnosis: ACS with troponin I positivity and angiographic triple-vessel coronary artery disease, in a patient with diabetes mellitus and hypertension.
