It looks like the top layer has been picked open. Do this now:
- Wash your hands, then gently rinse the spot with clean running water and mild soap. Do not scrub it or use alcohol, peroxide, toothpaste, or harsh antiseptics.
- Pat dry with clean gauze or tissue.
- Apply a very thin layer of plain petroleum jelly and cover it with a non-stick sterile dressing or bandage. Change it daily, or sooner if wet/dirty. Keeping a small wound clean, moist, and covered helps healing, per AAD wound-care advice.
- Do not pick, squeeze, or peel it again. Trim nails, cover it so you cannot feel the uneven edge, and use a cold compress over the dressing for 10 minutes if it is itchy.
- Remove or loosen the anklet if it rubs the area or if your ankle becomes swollen.
Because there is a fairly large dark area around it and the center is open, arrange an in-person medical assessment within 24 hours, especially if this began as a blister, bite, boil, burn, or unexplained sore.
Get urgent care today if you develop spreading redness, warmth, worsening pain or swelling, pus/bad smell, fever, red streaks up the leg, numbness, the dark area is enlarging, or you have diabetes, poor circulation, or reduced immunity. Those can be signs of infection or a deeper skin problem, as described in AAD infection warning signs.
