Elderly male who has been diagnosed of type 2 DM has been on OHa, tablet name. Unknown. He has currently undergone surgery for buccal mucosa cancer..pod8.. currently HbA1C is 6.9...sliding scale insulin requirement is approx 10 units per day max GRBS is 202 and minimum is 122..he is on rt feeds currently and is being discharged..what should be advice on discharge for diabetes and how to follow up for the same...creat is normal
