NICU fluconazole prophylaxis: 6 mg/kg/dose IV or oral twice weekly for 6 weeks, started within the first 4-72 hours after birth.
It is generally recommended for extremely low birth weight infants (<1000 g) in NICUs with a high invasive candidiasis rate, commonly defined as ≥10%. Use the IV route initially if enteral feeds are not tolerated; oral absorption is good once feeds are established. AAP Red Book guidance supports this regimen.
Some protocols use 3 mg/kg/dose twice weekly, particularly where lower-dose local protocols are used. Follow your NICU's antifungal policy and monitor liver enzymes/clinical drug interactions.
