Pulse ek arterial throbbing hai jo heart ke contractions se produce hoti hai. Isko examine karte waqt ye points note karte hain:

Rate Ke Anusaar Types

1. 🔴 Tachycardia (Tez Nadi)

  • Rate > 100/min (adults)
  • Kab hoti hai: Fever (bukhar), Exercise (vyayam), Anxiety (chinta), Hyperthyroidism, Shock, Anemia

2. 🔵 Bradycardia (Dheemi Nadi)

  • Rate < 60/min (adults)
  • Kab hoti hai: Athletes, Hypothyroidism, Heart block, Beta-blocker medicines

3. Regular Pulse (Niyamit Nadi)

  • Har beat equal interval pe aati hai
  • Normal hai

4. Irregular Pulse (Aniyamit Nadi)

  • Beats unequal intervals pe aati hain
  • Causes: Atrial Fibrillation (AF), Ectopic beats

5. 💥 Water-Hammer Pulse / Collapsing Pulse (Pani ki tarah uchhalti nadi)

  • Bahut tez rise aur tez fall
  • Brachial artery ko upar uthao toh aur zyada feel hoti hai
  • Cause: Aortic Regurgitation (AR), Thyrotoxicosis, Severe Anaemia, PDA

6. 🌊 Plateau Pulse / Anacrotic Pulse (Dhire uthne wali nadi)

  • Bahut dheere rise, peak pe ruk jaati hai
  • Cause: Aortic Stenosis (AS)

7. Bisferiens Pulse (Do-choti wali nadi)

  • Ek hi beat mein do peaks feel hoti hain
  • Cause: Combined Aortic Stenosis + Regurgitation, Hypertrophic Cardiomyopathy (HOCM)

8. 〰️ Dicrotic Pulse (Dicrotic Nadi)

  • Main beat ke baad ek secondary small beat feel hoti hai (diastole mein)
  • Cause: Low cardiac output states, Typhoid fever, Dilated Cardiomyopathy

9. Pulsus Paradoxus (Saans ke saath badalne wali nadi)

  • Inspiration ke time BP > 10 mmHg gir jaata hai, nadi kamzor pad jaati hai
  • Cause: Pericardial Effusion (tamponade), Severe Asthma, COPD

10. Pulsus Alternans (Baari-baari badi-choti nadi)

  • Ek strong beat phir ek weak beat - regular interval pe
  • Cause: Severe Left Ventricular Failure (Heart Failure)

11. Pulsus Bigeminus (Joda-joda nadi)

  • Do beats saath-saath aati hain, phir pause, phir do beats
  • Cause: Ventricular Bigeminy (Ectopic beats alternate karna)

12. Small Volume Pulse / Thready Pulse (Kamzor, patli nadi)

  • Barely feel hoti hai
  • Cause: Shock, Severe dehydration, Heart failure, Severe AS

13. Large Volume / Bounding Pulse (Uchhalti nadi)

  • Bahut strong feel hoti hai
  • Cause: Exercise, Fever, AR, Hyperthyroidism, Anemia

14. Pulsus Deficit

  • Radial pulse ki rate < heart rate (auscultation se)
  • Cause: Atrial Fibrillation

BP Kya Hai? (What is Blood Pressure?)

Blood pressure arteries ki walls ke against blood ka force hai. Isko do numbers mein measure karte hain:
  • Systolic BP - Jab dil squeeze karta hai (upper number)
  • Diastolic BP - Jab dil relax karta hai (lower number)

BP Ke Special Types

1. ✅ Normal BP (Samanya Raktachaap)

  • 120/80 mmHg - "Standard reading"

2. 🔴 Hypertension (Uchcha Raktachaap - High BP)

  • Systolic ≥ 130 mmHg ya Diastolic ≥ 80 mmHg (ACC/AHA)
  • Symptoms: Zyada baar headache, vision problems, chest pain (aksar koi symptom nahi hota - "silent killer")

3. 🔵 Hypotension (Nicha Raktachaap - Low BP)

  • BP < 90/60 mmHg
  • Symptoms: Chakkar aana, behoshi, kamzori

4. Orthostatic (Postural) Hypotension

  • Khade hone par systolic BP > 20 mmHg ya diastolic > 10 mmHg gir jaata hai
  • Cause: Dehydration, diabetes, old age, medicines

5. White Coat Hypertension (Doctor ke saamne High BP)

  • Doctor ke office mein BP high, ghar pe normal
  • Anxiety se hoti hai

6. Masked Hypertension

  • Doctor ke paas normal, ghar mein ya bahar high BP
  • Zyada dangerous hai

7. Isolated Systolic Hypertension (ISH)

  • Systolic ≥ 140, Diastolic < 90
  • Mostly elderly mein
  • Atherosclerosis se arterial stiffness ke kaaran

8. Isolated Diastolic Hypertension

  • Sirf diastolic badha hua, systolic normal
  • Young adults mein zyada

9. Resistant Hypertension (Ziddi High BP)

  • 3 ya zyada anti-hypertensive medicines ke baad bhi BP control nahi hota

10. Malignant / Accelerated Hypertension

  • BP bahut suddenly bahut zyada badh jaata hai (> 180/120)
  • Organs ko turant khatara - Medical Emergency!
  • Retinal changes (papilledema) dikhti hai

11. Secondary Hypertension

  • Kisi disease ki wajah se BP badha hua
  • Causes: Renal disease (gurde ki bimari), Pheochromocytoma, Hyperaldosteronism, Renal artery stenosis

12. Pulse Pressure (Nadi Chaap)

  • Systolic BP - Diastolic BP = Pulse Pressure
  • Normal: 30-40 mmHg
  • Wide pulse pressure (> 60): AR, Thyrotoxicosis, Atherosclerosis
  • Narrow pulse pressure (< 20): Severe AS, Cardiac Tamponade, Shock

13. Mean Arterial Pressure (MAP)

  • Formula: MAP = DBP + 1/3 (SBP - DBP)
  • Normal: 70-100 mmHg
  • Organ perfusion ke liye important
  • MAP < 65 = Shock ka sign
